    @ 12:30 p.m.: Client: Mary Sansone, 12 Bonnett Avenue, Larchmont, NY 10538 Telephone: 914-834-7388 Position Days/Hours: Saturdays and Sundays 9:30 a.m. to 9:30 p.m. Rate of Pay: $17.00 per hour @ 12-hour shift = $204.00 Breakdown: Medicaid Pays $13.00 per hour @ 8 hours = $104.00 Family to subsidize private pay for 8 hours @ $4.00 hour = $32.00 Also private pay for 4 hours @ $17.00 per hour = $68.00 Total Private Daily Pay: $100.00 As discussed, you will be paid by an Agency at the Medicaid Rate of $13.00 per hour for 8 DSS approved hours. (Details will be provided to you Monday or Tuesday of where and how to register. Agency is located in White Plains, NY. You must register no later than Wednesday, November10th since proposed start date will be Saturday, November 13th.)
