Pulsus Epigastricus

In patients with an increased anteroposterior (AP) diameter, palpation of the right ventricle in the epigastric or subxiphoid area is also useful. With your hand flattened, press your index finger just under the rib cage and up toward the left shoulder and try to feel right ventricular pulsations.

STEPS FOR ASSESSING THE JUGULAR VENOUS PRESSURE (JVP)

Make the patient comfortable. Raise the head slightly on a pillow to relax the sternomastoid muscles. Raise the head of the bed or examining table to about 30. Turn the patients head slightly away from the side you are inspecting. Use tangential lighting and examine both sides of the neck. Identify the external jugular vein on each side, then find the internal jugular venous pulsations. If necessary, raise or lower the head of the bed until you can see the oscillation point or meniscus of the internal jugular venous pulsations in the lower half of the neck. Focus on the right internal jugular vein. Look for pulsations in the suprasternal notch, between the attachments of the sternomastoid muscle on the sternum and clavicle, or just posterior to the sternomastoid. The table below helps you distinguish internal jugular pulsations from those of the carotid artery. Identify the highest point of pulsation in the right internal jugular vein. Extend a long rectangular object or card horizontally from this point and a centimeter ruler vertically from the sternal angle, making an exact right angle. Measure the vertical distance in centimeters above the sternal angle where the horizontal object crosses the ruler. This distance, measured in centimeters above the sternal angle or the atrium, is the JVP.

Jugular venous pressure measured at more than 4 cm above the sternal angle, or more than 9 cm above the right atrium, is considered elevated or abnormal.

JUGULAR VENOUS PRESSURE (JVP)

Vena jugularis interna mereflesikan tekanan atrium kanan. Hendaknya diobservasi ketinggian maksimum JVP, dan pulsasinya. a.Posisikan pasien dengan sudut 45 dan leher disangga agar otot leher rileks mungkin leher perlu diputar kelateral b.Observasi perbatasan otot sternokleidomastoideus dengan klavikula dan perhatikan alur vena jugularis, lihat pulsasinya.

c. Coba palpasi pulsasi, bila dapat dirasakan berarti kemungkinan berasal dari arteri karotid. Oleh karena pulsasi vena jugularis tidak mungkin diraba. Pulsasi vena jugularis biasanya kompleks dengan gelombang masuk (inward) , sedangkan pada arteri jugularis biasanya sederhana berupa gelombang keluar (outward) yang dominan. JVP biasanya menurun saat inspirasi d. Perkirakan ketinggian ertikel dari pulsasi kesudut manubriosternal. Vena jugularis eksterna biasanya lebih mudah dikenali, karena letaknya di later al otot sternokleidomastoideus dan lebih superfisial.

Peningkatan JVP biasanya menandakan : 1Gagal jantung 2Obstuksi vena cava superior 3Peningkatan volum darah (kehamilan, nefritis akut, kelebihan terapi cairan)

Perkusi Pinggang Jantung

Ditentukan terlebih dahulu garis parasternal kiri. Kemudian dilakukan perkusi kearah caudal mulai dari titik teratas garis tersebut, dengan posisi jari tengah sejajar iga. Yang dicari adalah perubahan bunyi sonor-redup. Batas ini normal terletak pada sela iga III kiri. Bila titik batasnya misal pada sela iga II, berarti pinggang jantung menghilang. Hal ini terjadi karena pembesaran atrium kiri.

Mengendalikan Palpitasi
Pasien hanya disarankan untuk mengurangi stres emosi dan stres fisik ketika sedang memonitor gejala-gejalanya. Frekwensi dari premature contractions dapat dikurangi dengan cara mengurangi stres, berhenti merokok dan mengurangi konsumsi kopi dan alkohol.
