Dead on Arrival Should get the death certificate at the RHU.

Babies delivered at home and brought to the hospital for cord care should get their birth certificate from the RHU. In case the patient died during transfer/along the way to the hospital they will get the death certificate from the RHU. The JBBMH will only issue medical certificate for patient transferred to other hospital Certification/Abstract and others will be provided by JBBMH. The JBBMH medical records will provide affidavit of loss for the year 2006 below. The JBBMH medical section will issue medical record from 2007 onwards. All records should be kept for 25 years. Unauthorized person should not be allowed to enter the medical record section. How to retrieve: Old patientswho wish to retrieve the medical records should fill up request form from the medical records head (Mrs. Flora B. Palima) signed by the request party. In case of claiming of birth certificate, parents will be the one to claim. If the relatives will claim, they should have authorized letter from the parents of the baby. The medical abstract/discharge summary will be provided 2-3 days if the patient s discharged while the confined is within 24 hours. All charts will be accomplished completely before discharged of the patients. No charts will be accepted by the billing section if it is incomplete especially the consulting physician. No PF will be issued the consulting physician if the chart is incomplete. Checklist should be attached to the chart so that the ward head nurse will know the information that needs to be completed. Logbooks to the medical section should be secured. Policy no. 1 Policy on Record Storage and Retention To avoid losses of medical charts it should not be brought out of the hospital. Upon admission, all entries in the chart should legibly accurately and completely.
