GENERAL GUIDELINES
1. Only candidates for elementary graduation (Grade 6 or 7) of the current school year are qualified to take the UST Entrance Test for High School (USTET-HS). 2. Submit the following: a. Accomplished UST Application and Grades Form (Clear photocopy of Grade 5 Report Card, and Grade 6 First Quarter Grades, or Grade 6 Report Card, and Grade 7 First Quarter Grades, may be submitted in lieu of the filling up of the Grades Details. If clear photocopy is to be submitted, original forms must be presented.) b. Two (2) 2x2 recent, identical photographs (Print and sign your name at the back.) c. Clear photocopy of Birth Certificate and Baptismal Certificate (Non-Catholic applicants should submit a certification indicating their religious denomination) d. Non-refundable Application fee of PhP 500.00

Applicants with incomplete requirements will not be entertained.

GUIDELINES (Grades Details)

1. Only the School Registrar or Principal can certify the Grades Form. 2. The final average grade for each year level in Math, English and Science should be recorded in the appropriate boxes. 3. Average all interrelated subjects taken up within the same year. e.g. [Earth Science (85.00) + Science and Technology (87.00) ] / 2 = Science Grade (86.00) 4. Grades must be stated in standard quantitative equivalent. (e.g. 85.00, 87.00) 5. This Grades Form is not valid without the school seal.
