Admission Policies and Procedures Section I: Admission Requirements Admission/enrollment to the institution is a privilege granted to those who qualify

under the criteria set by the Committee on Admission. After completion of the Perpetual Help Qualifying Admission Test and Interview, the following requirements should be presented to the Office of the University Registrar (O.U.R.): 1. Freshmen o Acceptance Letter (Issued by the Guidance Office) o Report Card (Form 138) Original and two (2) photocopies o Certificate of Good Moral Character Original and two (2) photocopies o NSO-certified Birth Certificate three (3) photocopies o Four (4) copies of recent 2x2 picture 2. Transferees o Acceptance Letter (Issued by the Guidance Office) o Transcript of Records Original and two (2) photocopies; or: Copy of Grades Original and two (2) photocopies Honorable Dismissal/Transfer Credential - Original and two (2) photocopies o NSO-certified Birth Certificate three (3) photocopies o Certificate of Good Moral Character (Original with 2 Photocopies) o Four (4) copies of recent 2x2 picture 3. Foreign Students o Study Permit from the Philippine Government o Acceptance Letter (Issued by the Guidance Office) o Transcript of Records Original and two (2) photocopies; or: Copy of Grades Original and two (2) photocopies Honorable Dismissal/Transfer Credential - Original and two (2) photocopies o Birth Certificate three (3) photocopies o Four (4) copies of recent 2x2 picture 4. Degree Holders o Official transcript of records with Special Order. For graduates of accredited schools, transcript of records must bear notation that the school is accredited by a duly recognized accrediting agency and that the school is exempted from applying for Special Order. o Photocopy of Marriage Contract (if married) o Four (4) copies of recent 2x2 picture o NSO-certified Birth Certificate three (3) photocopies 5. Old Students o Identification Card for validation purposes o Evaluation sheet signed by the Dean o Photocopy of Marriage Contract (if Married)
