GUIDELINES ON DISPENSING DANGEROUS DRUG

1.) The prescription received must be assessed for: a. Incomplete doctors information b. Incomplete patients information c. Incorrect dosage form d. The prescriber cannot be the patient e. Without doctors signature f. update issuance

2.) The OR request form must be accurately read. Meaning, the quantity that are requested and the total quantity of all drugs written on each prescription must be the same amount.

3.) The nurse who carried the dispensed drugs must be responsible to secure the personalized prescription or the yellow prescription therefore the pharmacy asst. who dispense the drug is responsible to remind the nurse with certain time limit of submission of the prescription.

4.) Providing each partial filling must be indicated on the prescription with signature of the dispensing pharmacy asst. or pharmacist. The total quantity dispensed in all partial filling does not exceed the total quantity prescribed, and no partial filling occurs after 3 months from from the date of issuance of prescription.
