1 pc 2x2 colored picture on a white background, w/ name tag and in uniform (for marine graduate) or plain white polo (for non-marine) 1 pc 1x1 latest ID picture Accident Insurance Policy Medical Certificate Advance Training in Fire Fighting (ATFF) (Same as PSCRB) and Medical Certificate and ECG tracing with reading Additional Requirements Notice of Award for OWWA scholars Guarantee Letter for company-sponsored trainees Prescribed Attire TCI (6.09) - Male: long sleeves polo with necktie Female: with blazer SSBT -Marine Deck Officer Type B Uniform BST, PSCRB & ATFF trainees must bring extra shoes, long sleeve shirt and pants for practical exercises. NOTE 1) Full payment of training fee is required for the issuance of Proof of Registration (POR). The POR must be presented to the trainer upon entrance to the classroom. 2) Classes proceed during scheduled and unannounced holidays. 3) Schedules and fees are subject to change without prior notice. 4) Special batches may be conducted subject to the minimum number of trainees and availability of trainers. 5) Cut-off time for chance trainee is 8:30 am
