AFFIDAVIT OF TRIBAL AFFILIATION

I, CARLOS A. BASILAN, married, of legal age, Filipino citizen, with residence address at Philex Mines, Tuba, Benguet, after having been duly sworn to in accordance with law hereby depose and state that: I am a member of the Kalanguya Tribe, an indigenous group of Ahin, Hungduan, Ifugao, having born to spouses BASILAN MADAPPEL and MAYAN ABUEL of Ahin, Hungduan, Ifugao, on October 28,1950; My parents are both of the Kalanguya Tribe of Ifugao so that by reason of place of birth and lineage, I am a full fledged member of the said indigenous group; Likewise, it is an accepted practice and so part of the culture and tradition of tribal Filipinos, especially that of the Cordilleras, the Ifugaos included, for children to use the first name of their father or immediate ancestor as family name; It is no surprise that in my personal and family records since childhood, I had been carrying BASILAN as my family name; and I am executing this affidavit to attest to the truthfulness of the foregoing facts for all legal intents and purposes; IN WITNESS WHEREOF, I have hereunto affixed my signature this 23rd day of January 2008 at La Trinidad, Benguet, Philippines.
