    Application forms can be obtained from any Police Station, or through the Internet from
    the Official Police website www.police.gov.cy
    Every application must be accompanied by:

    1. An official Receipt for the payment of the amount of 17,09. The payment could be made:
    (a) in cash at any Divisional Police Headquarters or at any Embassy or Consulate of the
    Republic of Cyprus,
    (b) remittance to the Central Bank of Cyprus in the name of the Chief of Cyprus Police
    (SWIFT CODE: CBCYCY2NACC, Government General Account, account
    CY16001000010000000006001010) only in Euro, or
    Identity Card /Passport /Alien Book /Pink Slip or Yellow Slip.
