Background survey A healthy person is defined as a person who is not obviously ill, and whose physical and mental

functions correspond to those of the average person of the same sex in the same age-group. By including weight and height in my question I can make certain inferences about the student BMI (Body Mass Index).
Obesity is a medical condition in which excess body fat has accumulated to the extent that it may have an adverse effect on health, leading to reduced life expectancy and/or increased health problems. Body mass index (BMI), a measurement which compares weight and height, defines people as overweight (pre-obese) when their BMI is between 25 kg/m2 and 30 kg/m2, and obese when it is greater than 30 kg/m2. Colour blindness or colour vision deficiency is the inability to perceive differences between some of the colour that others can distinguish. It is most often of genetic nature, but may also occur because of eye, nerve, or brain damage, or exposure to certain chemicals. The English chemist John Dalton published the first scientific paper on the subject in 1798, "Extraordinary facts relating to the vision of colours", after the realization of his own color blindness. The point of this survey is to find out what is the state of health the student in

IPTHO is having. Problem statement In this world we can find all sorts of people with certain characteristic such as wear glasses, colour blind, thin or obese people. The form is given to a few classes so that they can tell what are their weights, height, eyesight state and what their common illnesses are.

Objective of survey To know certain characteristic of students in IPTHO To find out why students possess certain characteristic To respect other differences

Importance of Survey
What will you observe if you measure the heights and weights of all the student in your class? You will get a range of values from the shortest to the tallest and from the lightest to

the heaviest. By doing this I can find average weight of the student and make conclusion regarding their health

Limitation of Survey
The limits of my survey are 50 students. The respondents for my questionnaires are 50 students. This survey takes place in Institut Perguruan Tun Hussein Onn. Not everyone in IPTHO that involve in this survey even the surveyors is randomly chosen.
