Fish Bone Diagram: A Fish Bone Diagram is a diagram that is used for analyzing the relationship between problem

and its cause. Fish Bone Diagram is also known as Cause and Effect Diagram or Ishikawa Diagram. Kaoru Ishikawa created Fishbone Diagram. He was an original founder of the total quality management movement, designed this tool for the business community. The name given to diagram was fish bone because its figure looks like a fish skeleton. In this visual or graphical display, the spine of the fish is a line that leads to the outcome of given problem statement. Categories branch outward from the central stem. Side categories are major inputs that influence the outcome. Each of the side branches is where the root causes or actual drivers of a problem lie. Right fact plays a major role in the successful application of fish bone diagrams.

Objectives: The main objectives of this diagram are: Analyzing the main causes of a problem. Identifying and focusing on particular issues without resorting to complaints and irrelevant discussion. Identifying areas where there is a lack of data.

Steps/Method: 1-Clearly mention and define the problem to analyze the outcome. 2- Use a chart pack positioned so that everyone can see it, draw the spine and create the effect box. 3- Identify the main causes contributing to the effect being studied. 4- For each major branch, identify other specific factors which may be the causes of the effect. 5- Identify increasingly more detailed levels of causes and continue organizing them under related causes or categories. This can do by asking a series of why questions. 6- Analyze the diagram and identify causes that warrant further investigation. Since Cause-and-Effect Diagrams identify only Possible Causes. So Pareto Chart is used to determine the cause to focus on first.
