    Teacher Evaluation Form This form is for you to anonymously communicate your experience with what works best

    for you and your learning in the classroom. Please be as honest pas possible; you have a say in how you are taught and how you learn! Also, please be as specific as possible, so Mr. Wagner has a clear understanding of your unique learning needs.

    How would you rate Mr. Wagners overall performance from last semester when he taught for two weeks? 1 being very unhelpful, 5 being very helpful. 1 2 3 4 5

    Do you learn best working on quiet, individual activities or in groups, working collaboratively on an activity? Make sure you think about whether you like one better and whether you learn better.

    Can you think of some activities (journals/quickwrites, reading, essay-writing, discussions, etc.) in English or other classes that you feel you really learned a lot from and describe them?

    What kinds of activities do you feel you learn the least from?

    Is there anything else that you feel would help you keep focused and learn more in the classroom (seating arrangements, features of the room, or anything else at all)?

    Do you feel that you are capable of being successful and have all the tools that you need to be successful in this class for the rest of the school year? If not, explain briefly.
