POSITION DESCRIPTION Position Title:Social Worker Reports To: Executive Director

Basic Functions: Under the supervision of the Executive Director, provide social services in all its many aspects to the children and their families that come to Sankofa for assistance and that are involved with any activities provided by the agency. Examples of Duties & Responsibilities: Must be able to communicate effectively in both, written and oral form. Responsible for coordinating supportive services, identifying special guest to present workshops relevant to supportive services for youth & families that promotes self sufficiency. Conduct assessments with parents in their homes, on the phone and/or on site (if needed) Interpret and present Sankofas programs/projects to stakeholders, philanthropists, foundations, schools, community/faith-based organizations, etc... Identify and understand the scope of community resources and how they can meet the needs of our youth and their families and provide outreach activities to recruit community youths. Attend all meetings and staff development trainings Work with youth and their families to navigate the process by partnering with pro-bono attorneys and community volunteers Provide confidential individual counseling and follow-up on an on-going basis. Other duties deemed necessary. Qualifications: College degree preferred Knowledge and ability to work with families on all levels Knowledge in Microsoft software to prepare flyers, notices and newsletters Ability to communicate ideas in written and oral form Excellent time management and team building skills
