    Please outline your transport and care arrangements below. This form MUST be returned to Gavin Holburn either prior to the Championships by email (only) (gavin.holburn@det.nsw.edu.au), or submitted in person prior to commencing the first mornings play. Failure to do so will result in exclusion from the event.
