Weekly check-ins with National Office Identification of top three metrics used by Health Leads as proxies of clinical integration Sort desks into quartiles to understand current clinical integration landscape Review current analysis on this data to identify staff/desks from each quartile to interview further about trends in practices Develop a set of criteria that serves as foundation for clinic snapshot: delineated clear physical space, positively engaged clinic champions, continuous volunteer coverage Interviews with staff, clinic partners, patients Synthesis of learning and development of materials to present to HL sites
