This vitamer is one of two active coenzymes used by B-12 dependent enzymes in the body, and is specifically the B-12 form used by 5methyltetrahydrofolate-homocysteine methyltransferase (MTR), also known as methionine synthase. Methylcobalamin is notable for being one of the few examples in nature of a bona fide organometallic bond. Methylcobalamin has been studied in conjunction with sleep-wake rhythm disorders, where it appears to yield benefits, but at a low or inconsistent level. It is used in treating diseases of vitamin B12 deficiency (such as pernicious anemia), or diseases of effective B12 deficiency, such as vitamin B12 metabolic pathway pathologies. One study suggests that once absorbed, methylcobalamin may be retained in the body better than cyanocobalamin.

If the deficiency is not corrected, it can damage the nerve cells. If this happens, vitamin B12 deficiency effects may include: tingling or numbness in fingers and toes difficulty walking mood changes or depression memory loss, disorientation, and dementia

Although it is not a cure, Pramipex eases the symptoms of Parkinson's disease, a progressive disorder marked by muscle rigidity, weakness, shaking, tremor, and eventually difficulty with walking and talking. Parkinson's disease results from a shortage of the chemical messenger dopamine in certain areas of the brain. Pramipex is believed to work by boosting the action of whatever dopamine is available. The drug can be used with other Parkinson's medications such as Eldepryl, Sinemet, and Larodopa.

If you are taking Sinemet or Larodopa, Pramipex may allow a reduction in your dosage. What side effects may occur?


Side effects cannot be anticipated. If any develop or change in intensity, inform your physician as soon as possible. Only your physician can determine if it is safe for you to continue taking Generic Mirapex. * More common side effects may include: Abnormal dreams, arthritis, chest pain, confusion, constipation, decreased sensitivity to touch, difficulty breathing, difficulty walking, dizziness, dizziness upon standing, drowsiness, dry mouth, hallucinations, increased muscle tone, increased urination, insomnia, involuntary movement (jerky motions), lack of appetite, memory loss, nasal inflammation, nausea, swelling, urinary tract infections, vision abnormalities, weakness * Less common side effects may include: Decreased sex drive, delusions, difficulty swallowing, fever, general feeling of illness, impotence, inability to hold urine, muscle spasms or twitching, pneumonia, skin disorders, thinking abnormalities, uncontrollable restlessness, unfounded suspicions, weight loss * Rare side effects may include: Abnormal ejaculation, abnormal heartbeat, agitation, blood clots, blood in urine, blood circulation problems, convulsions, difficult or painful urination, enlarged abdomen, eye disorders, heart attack, heart problems, joint problems, lung problems, mental illness, muscular problems, prostate problems, severe chest pain (angina), thirst

Special warnings about this Pramipex


Pramipex (Pramipexole Dihydrochloride, Generic Mirapex) can cause your blood pressure to drop when you first stand up, resulting in symptoms such as dizziness, nausea, fainting, blackouts, and, sometimes, sweating. To avoid or reduce these symptoms, try to stand up slowly, especially at the beginning of treatment with Pramipex. Pramipex can cause drowsiness and may trigger hallucinations, especially if you are over 65 or have an advanced case of Parkinson's. You may even fall asleep--without warning and without feeling drowsy--while performing everyday activities. Check with your physician immediately if you find that you're getting drowsy or falling asleep while eating, talking, or watching television. Do not drive a car or undertake other dangerous activities until you've spoken with the physician. Be especially cautious when taking other drugs that cause sleepiness. If you have a kidney condition, make sure the physician is aware of it. You'll probably need regular blood tests to check your kidney function, and your dosage of Generic Mirapex may have to be reduced. In very rare cases, Pramipex (Pramipexole Dihydrochloride, Generic Mirapex) may cause muscle wasting. If you develop muscle aches or soreness after you start Pramipex, be sure to tell your physician. Also alert your physician if you notice any changes in your eyesight.

Possible food and drug interactions when taking this medication


If Pramipex is taken with certain other drugs, the effects of either could be increased, decreased, or altered. It is especially important to check with your physician before combining Pramipex with following: Carbidopa/Levodopa (Sinemet) Sedatives and tranquilizers such as chloral hydrate, codeine products, Dalmane, Halcion, and phenobarbital Cimetidine (Tagamet) Diltiazem (Cardizem, Dilacor XR) Major tranquilizers such as Compazine, Haldol, Mellaril, Navane, Prolixin, Stelazine, and Thorazine Metoclopramide (Reglan) Quinidine (Quinidex, Quinaglute) Quinine Ranitidine (Zantac) Triamterene (Dyrenium) Verapamil (Calan, Isoptin) Combining Pramipex (Pramipexole Dihydrochloride, Generic Mirapex) with Sinemet or Larodopa sometimes triggers twitching and jerky movements. If this happens, tell your physician. A reduction in your dose of Sinemet or Larodopa may solve the problem.

Special information if you are pregnant or breastfeeding


The effects of Pramipex during pregnancy have not been adequately studied, so it's best to avoid it if you're expecting. If you are pregnant or plan to become pregnant, inform your physician immediately. It is not known whether Pramipex appears in breast milk. If the drug is considered essential to your health, your physician may advise you to stop breastfeeding while taking the medication.

Recommended dosage
ADULTS The usual starting dose is 0.125 milligrams 3 times a day. If necessary, your physician may increase the dose every 5 to 7 days until the maximum dose of 4.5 milligrams a day is reached. Dosage is usually increased gradually to minimize the drug's potential side effects. If you have kidney disease, the physician will keep the dosage quite low. CHILDREN The safety and effectiveness of Pramipex (Pramipexole Dihydrochloride, Generic Mirapex) in children have not been established.

Why do I need this medicine? NEXITO is used to treat depression. It can also be used to treat anxiety disorders such as panic disorder, social anxiety disorder or obsessive-compulsive disorder (OCD). NEXITO belongs to a group of antidepressants known as Selective Serotonin Reuptake Inhibitors (SSRIs). How do I take this medicine? Take NEXITO exactly as directed by your doctor. Do not take more or less than instructed by your doctor. You may need to take NEXITO for some weeks before the full benefits can be felt. Do not be discouraged if you do not feel better soon after taking the medicine. NEXITO must be taken regularly for it to work well. Do not stop taking NEXITO unless instructed by your doctor. You may feel unwell if NEXITO is stopped suddenly. When your doctor decides that you no longer need NEXITO, he will usually reduce your dose gradually before stopping it. Follow the doctor's instructions carefully. You may take NEXITO with or without food. Try to take it at about the same time each day. NEXITO may be available as tablets or as oral drops (solution). If you have been given the tablet, swallow it with a glass of water. If you have been given the oral drops (solution), use the dropper provided to measure out your dose accurately. Dilute your dose in a glass of water, apple or orange juice (about 120mL) just before you take it. After dilution, drink it immediately. What should I do if I have forgotten to take this medicine? Take the missed dose as soon as you remember. If it is almost time for your next dose, skip the missed dose and return to your normal dosing schedule. DO NOT double a dose under any circumstances. If you often forget to take your medicine, let your doctor and pharmacist know. When should I not use this medicine? Do not take NEXITO if you are taking or have taken other mood medicines known as monoamine oxidase inhibitors (MAOIs) in the last 14 days. Some examples of MAOIs include isocarboxazid, phenelzine, moclobemide, selegiline and tranylcypromine. Alert your doctor immediately if you are taking or have taken any of these medicines or other MAOIs. Alert your doctor if you are taking an antipsychotic medicine known as pimozide or an antibiotic known as linezolid. What should I take note of while using this medicine? Alert your doctor if you are pregnant or breastfeeding. Do not breastfeed while being treated with NEXITO. Alert your doctor if you suffer from any of these conditions: - mood disorders including bipolar disorder (mania and hypomania)

- diabetes - epilepsy (fits) - liver or kidney disease - heart disease, recent heart attack or an abnormal heart rhythm - a history of bleeding disorders Alert your doctor if you are undergoing electro-convulsive therapy (ECT). NEXITO is not meant for children and teenagers younger than 18 years old. Antidepressants such as NEXITO increase the risk of suicidal thoughts and behaviour in children, teenagers and young adults. As depression and some psychiatric disorders are themselves associated with an increased risk of suicide, it is important for you or your family members to monitor your condition especially at the start of treatment and if your dosage is still being adjusted. If you experience new or sudden changes in mood, behaviour, thoughts or feelings or a worsening of your depression with suicidal thoughts or attempts, harm to self or harm to others, contact your doctor immediately. What side effects could I experience? NEXITO may cause drowsiness, dizziness or blurred vision. If you are affected, do not drive or take part in any activity in which you need to be alert or be able to see clearly. Dizziness may be worse especially when you have been standing for some time. Avoid standing still at one spot for too long, always move your legs around. Other common side effects of NEXITO include nausea, dry mouth, indigestion, stomach discomfort, stomach wind, diarrhoea, constipation, weight gain, headache, difficulty sleeping, unusual dreams or nightmares, ejaculation delay, tiredness, weakness, sweating, joint pain, muscle ache, agitation and nervousness. Alert your doctor if any of these side effects are severe or refuse to go away. Other side effects are less common but may need medical help. Alert your doctor if you develop any of these symptoms: - hallucinations - unusual restlessness and the inability to rest or sit still - agitation or extreme, unexplained excitement (non-stop talking, moving or gesticulating) - very fast, irregular or pounding heartbeat - difficulty breathing - severe muscle spasm or stiffness Can I take this with other medicines? Do not take NEXITO if you are taking or have taken other mood medicines known as monoamine oxidase inhibitors (MAOIs) in the last 14 days. Some examples of MAOIs include isocarboxazid, phenelzine, moclobemide, selegiline and tranylcypromine. Alert your doctor immediately if you are taking or have taken any of these medicines or other MAOIs. Alert your doctor if you are taking any other medicines, especially those listed here: - mood medicines and other antidepressants such as venlafaxine, desvenlafaxine, nefazodone, milnacipran, buspirone - blood-thinning medicines such as warfarin, clopidogrel, dipyridamole or ticlopidine - painkillers of the NSAID type such as ibuprofen, diclofenac, naproxen, aspirin - other painkillers such as tramadol

- medicines to treat mental illness such as chlorpromazine, haloperidol, thioridazine, lithium - isoniazid (tuberculosis or TB medicine) - antifungal medicines such as fluconazole - an antibiotic known as linezolid - diet pills such as sibutramine - hay fever medicines such as astemizole - carbamazepine (epilepsy medicine) - migraine medicines such as sumatriptan, naratriptan, zolmitriptan or eletripan - stomach medicines such as metoclopramide, cimetidine, omeprazole, esomeprazole, lansoprazole - St John's wort or tryptophan (herbal remedies) Always inform your doctor or pharmacist if you are taking any other medicines, including herbal tonics, supplements and medicines that you buy without a prescription. Are there any restrictions on the type of food I can take? How should I store this medicine? Avoid alcohol. Alcohol, even in small amounts, can worsen the drowsiness caused by NEXITO. It can also affect your reaction time and make it unsafe for you to drive or take part in activities in which you need to be alert. Store in a cool, dry place away from the reach of children. If you have been given the oral drops (solution), throw it away 8 weeks after you first open it. Write the date of opening on the bottle so that you can keep track of when it should be thrown away. Medicines must not be used past the expiry date.
