Instructions: * Use "Internet Explorer" to Download Application Form. (Recommended) * Use Capital Letters to fill the registration form. * Fill your information carefully and proof read it before submitting your Online Application. * Carefully select your desired test city and desired subject, you can not change once you have
submitted the application form.

* Click the submit button only once, application submission can sometimes take few minutes if you
are using a dialup connection, so be patient. this form and deposit slip.

* After successful registration, application form will be displayed with online bank deposit slip. Print * You can deposit fee in any branch of ABL, MCB, HBL or UBL. * Send application form to NTS, after attaching Deposit Slip (NTS Copy), 2 Passport size
photographs and CNIC (Copy). Manager Operations National Testing Service 96, Street No. 4, Sector H-8/1 Islamabad * Fill your own registration form and send it to NTS, if you copy the registration form of any other candidate, both registration form will be rejected.

* Please make sure that the "Deposit ID", "Candidate Name" and "CNIC Number" should
be same on the registration form and deposit slip. "Do not use the deposit slip / registration form of any other candidate".

* Please use one postal envelope for one registration form, do not send more that one
registration form in one postal envelope, otherwise registration form will not process.

* Application without the original deposit slip (NTS Copy), 2 Photographs and CNIC (Copy) will not
be entertained.

* Keep a photocopy of the deposit slip with you for your record.
