TRAINING AGREEMENT AND LIABILITY WAIVER

I, Myca A. Breboneria, an Office Administration student of Camarines Sur Polytechnic Colleges, Nabua, Camarines Sur, hereby voluntarily agrees to undergo on-the-job training, a mandatory requirement for entrance to the degree programs of the course BSOA, under the following conditions: a. That I shall abide all Social Security System (SSS) rules and regulations and shall comply with those imposed for the training, otherwise, I shall be excluded for further participation in the in-company training. b. That there is no labor-management relationship between me and Social Security System (SSS).

c. That I shall exercise care and diligence in any task assigned to me in the personnel office. d. That I shall denounce and waive my claim against Social Security System (SSS), and Camarines Sur Polytechnic Colleges for any loss that I may sustain or loss that I may suffer, personal and pecuniary, in the performance of my duties and functions while under on-the-job training.

e. That I shall be responsible in any case of liabilities or damage to property or injury to third person which may be occasioned by international or negligent acts while in the course of on-the-job training.
