REPORT ON INVESTIGATION OF ALLEGED BURST PIPELINE AND SPILLAGE SITE OPPOSITE AJAYI FARM ABEOKUTA EXPRESSWAY IKEJA LAGOS

BRIEF: There were radio announcements and text messages reporting an impending fire accident that may arise from petroleum spillage hazard in Ajayi Farms area of Abeokuta Express Way Ikeja Lagos. OBSERVATION: As at the time of visit, 2:20pm, the alleged spillage site is on the Abeokuta highway of Ikeja next to the rail line. There was no visible petroleum or gas pipeline within the vicinity. However, there was a patch indicating spillage of petroleum product on a small section of the highway. Petroleum stench pervaded the area. People were scooping kerosene from the drainage beside the express way. Almost all the kerosene floating on the drain water had been removing leaving only the smear. A considerable portion of the spilled kerosene had been scooped out of the drainage. However, film visible all over the drainage. ACTION TAKEN Information gathered from motorcycle riders and passers-by revealed the followings: At the early hours of Saturday 16th June, 2012, a pathfinder car had a head on collision with a moving kerosene laden tanker consequently rupturing one of the units of the tanker thereby spilling kerosene onto the highway and drains. The tanker drivers could not prevent the spillage as the primary concern was to rescue the occupants of the car for prompt medical attention. It was reported that two of the passengers died on the spot while the other 2 were taken to General Hospital Ikeja. Submitted please.
