Music Focus Group
Permission Form
Students in GT 8-9 have the option of participating in a music focus group that
meets on Tuesdays in the Band Room at Helena Middle School. Students will need to bring their own
instruments or obtain permission from Mrs. Nielsen to use a school instrument.

This group will meet from 2:45 – 3:45.

Students are responsible for arranging their own transportation.

Students must have parental permission to attend this focus group. Signing this form will serve as
permission for your student to participate in this activity for the 2008-09 school year.
