Cables could get in the way of the camera, tripod, actors, and cause injury to the cast and crew

Control what are you doing to minimise the risk?


Two Production Assistants Levi De Sousa and Crimson Wright will be tasked with taping down wires using the provided gaffer tape and will act as Grips where necessary e.g. dolly shots

Communication what efforts are you making to communicate the risk to members of the team, the general public etc?
Meeting before the scenes to detail potential risk and for all cast and crew to be vigilant for potential accidents. The music video shoot is in a marked off space from the general public so there will be no interference. Furthermore, this Risk Assessment Form and all other documentation has been displayed on a Blog for all cast and crew to see 2 months prior to the shoot: http://ndkmusicvideo.tumblr.com/

The whiteboard with the black hole needs to be securely positioned so it does not fall on the cast or crew. The wheelie chairs needs to be used with care as we cannot risk our actors falling off and hurting themselves on the Hall floor/ marking up the Hall floor The somersaults that the actors will be undertaking will need to be cushioned by a series of securely laid down gym mats (provided) and tested prior to the shoot

The whiteboard will be securely positioned by myself with a stand and monitored throughout the shoot The wheelie chairs and gym mats will be laid down by Levi De Sousa and Crimson Wright with supervision from myself.

Communication what efforts are you making to communicate the risk to members of the team, the general public etc?
This will be addressed to to cast and crew in a meeting prior to the shoot of the scenes in question

The make-up may cause an unwanted reaction on the actresses skin The costume will restrict performance/ get torn on the props during the tasks

Control what are you doing to minimise the risk?


Kimberly Veck and /Chlore Ridout will conduct make-up tests on the girls skin prior to full application. The actresses have also been asked if they have any allergies relating to make-up and none have any issues so we will be covered. The actresses are supplying their own costume and will be made fully aware of what they are doing in each scene prior to the shoot. If their costume is damaged at all I will pay to have it repaired/ replaced.

Seadhna or Nic fall off their wheelie chair in the race Sacha or Nic injure themselves engaging in their somersault race

Nic is hurt by the girls as they push him back

All actors will have time to rehearse/ walk through the scene with me and will be told that they cannot go too fast as they may fall off/ break the chair/ injure themselves/ mark the floor. The actors will have precise, clear, directions at every stage.

Meetings/ walk-throughs with Director, Rehearsal

Actors may get carried away in the scene and cause injury to their co-performers

Speaking to each Actor via social media building up to and on the day of the shoot to explain the needs/ marks/ aims of their performance, and to walk-through with them how to execute their role

3 of the actors are under 18 at the time of recording and so will have the requisite amount of breaks, lunch-time, support, and permission from their parent and/ or guardian for the shoot

I am working closely with the actors (also my students) to ensure they are fully supported and have created a Contributor Release Form for their parents and/ or guardians to sign before we release the finished project online
