First attempt made within two business days of receiving service copies. Standard service includes three (3) valid attempts to serve, a notarized affidavit of proof of service, and we notify you immediately when service is complete and then file the proof with the appropriate court. Substitute service mailing available when you request and provide additional copies. Special instructions for service must be in writing, along with the request for service form. No refunds on attempted service. A not to exceed on total fees must be stated on form to apply. Additional mileage fees are not required unless the service address is more than 40 miles round trip from the Lebanons DMV office. Make sure to include any additional fees when applicable.
