In the northern part of Iraq, an elderly priest called Father Merrin runs an archaeological dig where he studies ancient relics. When he discovers the statue of an ancient demon mysteriously juxtaposed together with a contemporary St. Joseph medal, a sequence of omens warns him of an imminent confrontation with some sort of powerful evil.

Meanwhile, in America, a young girl called Regan MacNeil who lives with her mother in Georgetown falls extremely sick whereby she goes through physical as well as psychological disorders and appears to be possessed by a demon. After various efforts for Regans treatment that yield no success, her mother approaches a local preist named Father Karras. Father Karras who himself is going through a troubled phase agrees to treat Regan but initially turns down the belief that she is possessed by an actual demon.
