AFFIDAVIT
I, MARTINA DELA VILLA BILLATE, of legal age, Filipino, widow, and a

residents of Brgy. San Policarpo, Calbayog City after having been duly sworn to in accordance with law, hereby depose and say: 1. That I am the wife of the late ARIEL CAGOMOC BILLATE who was born on May 03, 1983 in Manila to spouses Bonifacio R. Billate and Yolanda J. Cagomoc; 2. That he was baptized at the Parish of Sto. Domingo, Quezon City on July 24, 1983 and he was given first name as ARIEL and had been using such names for all intent and purposes ; 3. That upon verification of his birth, he was registered in the National Statistics Office but without first name appearing in his certificate of birth; 4. That I am attesting the truth that the complete name of my husband is ARIEL CAGOMOC BILLATE and had been using in all his pertinent documents during his lifetime. 5. That finally I am executing this affidavit to attest to the truth and veracity of the foregoing facts as well as for all lawful purposes the same may serve.
