Good Morning, My original request was made on August 14, 2012 via email, 44 days ago, and I hereby give notice that if the requested information is not received within 7 days (October 4, 2012), action will be taken to forward this request to the State of Florida for their investigation and action. I would like to follow up by making my 4th request for the Council Information Request: The following information has been requested for the Mayor's, and all members of his staff's, expenses (including contractors to the City) (in accordance with IRS requirements) please provide the business purpose & attendees for each expense. Please include expense reports and pdf's for all filed receipts as well as detail for any city credit cards, personal credit cards, or other reimbursed expenses. Also, please provide a PDF of each receipt submitted. 1. FY '11 and YTD FY '12 Acct. listing of all purchases by the Mayor and details for SPECIFIC budget authorityplease provide a listing from the Purchasing Department. 2. FY '11 and YTD FY '12 expense reports with detail for Mayor Hayward, including all Mayor's Staff members & contractors. In addition please provide all expenses paid by others on behalf of the Mayor or his staff, including airfare, event passage, gifts and meals, lodging, or any other expenses paid for by any third party. Finally, I would like to see all City credit card statements for all city credit cards irrespective of holder from 01/01/2011 through the date of this request. Please provide in PDF format. Please note, I have discussed this request with the CFO on several occasions and it was promised that as documents within this request are available each will be forwarded.
