LOCAL OPTION ELECTION Within Calcasieu School District: Shall the number of terms of office that any member of the school board may serve be limited to three consecutive fouryear terms? o YES o NO One of Calcasieu Parish, Louisiana, renew, levy and collect a tax not exceeding 7.00 mills for a period not to exceed ten (10) years beginning in 2013, in excess of and in addition to other taxes levied by the District, for the purpose of acquiring, constructing, improving, maintaining or operating fire protection or emergency medical service facilities, vehicles and equipment, both movable and immovable property, owned or used by the District directly or indirectly to provide fire protection ot emergency medical service within the District, including without limitation, salaries and benefits of fire district personnel, the cost of obtaining water, communication, equipment, uniforms and other materials and supplies, the cost of fire hydrant rentals and service, the cost of fuel, insurance and maintenance for vehicles and equipment, the cost of insurance, utilities and upkeep of buildings and grounds, and all other maintenance and operating costs, title to which shall be in the public? Summary: 10 year, 4 mills property tax for improving, maintaining and supporting the Allen Parish Courthouse and Jail. Shall the Parish of Allen, State of Louisiana (the Parish), levy a special tax of four (4) mills on all the property subject to taxation in the Parish (an estimated $308,000 reasonably expected at this time to be collected from the levy of the tax for an entire year), for a period of ten (10) years beginning with the year 2013 and ending with the year 2022, for the purpose of improving, maintaining and supporting the Courthouse and Jail of the Parish? o YES o NO hundredths of a mill (.18) increase over the 5.12 mills tax authorized to be levied through the year 2012 pursuant to an election held on November 5, 2002? o YES o NO Wds./Dists./Pcts.

Summary: Authority to issue $400,000, 15 year general obligation bonds for acquiring, constructing and improving fire protection facilities and equipment, including fire trucks for said District, to be payable from ad valorem taxes. Shall Fire Protection District No. 6 of the Parish of Allen, State of Louisiana (the District), incur debt and issue in one or more series from time to time not exceeding Four Hundred Thousand Dollars ($400,000) of general obligation bonds to run not exceeding fifteen (15) years from the date thereof, with interest at a rate or rates not exceeding eight percentum (8%) per annum, to be sold at par, premium or discount, for the purpose of acquiring, constructing and improving fire protection facilities and equipment, including fire trucks, for said District, title to which shall be in the public, which bonds will be general obligations of the District and will be payable from ad valorem taxes to be levied and collected in the manner provided by Article VI, Section 33 of the Constitution of the State of Louisiana of 1974, and statutory authority supplemental thereto, with an estimated 3.85 mills (an estimated $47,043.00 reasonably expected at this time to be collected from the levy of the tax for an entire year) to be levied in the first year of issue to pay said Bonds? o YES o NO Wds./Dists./Pcts.
