1.
- Improper lanyard anchorage. - Improper wearing of fall protection - material fall hazard
- Personal injury
- Ensure lanyard is hook properly, Hook to rigid structure - Check Harness from cuts, wear and tear - Harness worn properly and adjusted. - Remove defective harnesses out of service
M - Conduct awareness -Proper wearing harness and correct way how to use.

- Use only scaffolding with green tag - Wear Fall protection - Lock caster wheels prior to work on the platform - mobile scaffold to follow 1:4 base height ratio
- 3rd party inspection - Conduct awareness -proper of wearing harness in correct way to use.

- Ladder
- Missing guardrail - Removing of Guardrails deliberately due to required space requirements. - Stockpiled tools and other materials along walk way/access - Floor Openings -material fall hazard - Defective ladder - Improper use of ladders - Throwing of materials and tools
- Personal injury
- Install temporary guardrails do not leave openings. - Ensure approval when removing existing/permanent structure or guardrails. - Avoid storing unnecessary materials on the access - Ensure toe boards installed - Use proper lifting material basket - Inspect ladder before use
- Never use nylon rope or barricade tape as protection on open edge. - When ladder is required to be vertical proper lashing and clamp must be required.
- Observed 3 point contact while using ladder - Install ladder 4:1 ratio

- Personal injury/fatality
- Wear full body harness - Install temporary covering on floor openings. - If barricade is not ideal, provide life line connected to rigid structure at both end.

- Man basket
- Basket failure ( welding on lifting joints) - High wind - Acrophobia (fear of Height)
- Scaffolding platform
- Incomplete guardrails
- Personal fall
- DPT test on lifting eyes. - Do not operate if the wind speed is more than 15 miles per hour. - Monitor wind speed - Do not allow/force person to work at high elevation if having acrophobia. - communication between ground/crane operator and man basket personnel - Wear Fall protection - Install auxiliary sling for lifeline purpose.
- No work at nights - Basket and rigging to be inspected by competent person - Follow TRA of Flare tip replacement activities
