9. Weekly Schedule for Practicum Hours: Tuesdays 5pm 8pm, Wed 3pm 9pm, Thursday 5pm 9pm, and Friday 10 4pm 10. Total Hours. This practicum involves a total of 240 hours to be completed between September 8 and December 11. Practicum hours must be scheduled relatively evenly each week throughout the academic term. No time should be scheduled during University holidays or winter break unless approved in advance by the Faculty Liaison & MSW Field Coordinator. 11. Supervisory Meetings. A weekly supervisory meeting between the student and the field instructor will be scheduled each Wednesday at 3:30pm. 12. Learning Goals and Objectives: Specify who is responsible Specify what will be accomplished Indicators (how accomplishment will be documented/measured) Timeline (objective to be accomplished within what time period) Work Plan (include if not apparent from description of objective - it describes action steps needed to accomplish objective)

Goal 1: To demonstrate the ability to perform the entry-level social work role of Advocate by applying social work values, ethics, and legal mandates while working with South Peninsula Haven House residents. Objective 1.1: Attend Haven House Advocate Training.

By October 1st, I will have attended Haven Houses Three Day Advocate Training By October 1st, I will be trained in creating Safety Plans By October 1st, I will be trained on crises hotline protocols. By October 1st, I will be trained on Shelter Policies and Procedures.

Objective 1.2: Observe and Assist With Three Shelter Client Intakes. By October 1st, I will learn the policies and approaches to client intake. By December 1st I will have observed a shelter advocate; gather information to determine clients eligibility for service, describe Haven houses policies of confidentiality, full disclosure, releases of information, and mandatory reporting. Objective 1.3: Assist With Three Night Shifts As Shelter Advocate. By December 1st I will have shadowed three different shelter advocates. By December 1st, I will have helped cover agency hotline for three shifts. By December first I will have assisted with nightly duties on three different occasions; such as cooking dinner, checking in on residents, and any other shelter operational needs that arise. Objective 1.4: Engage with Shelter Residents By December 1st, I will attend 5 Shelter House Meetings I will Interact with shelter residents on a weekly basis. I will attend at least two shelter dinners a month. Indicators: Use and evaluation of Advocate skills will be discussed during clinical supervision. The shelter manager and shadowed advocates will assess abilities and evaluate performance at the end of shift. Shifts worked will be documented in weekly timesheets; signed by field instructor. Documentation will be included in learning portfolio.

Goal 2: To acquire hands on experience conducting effective social work groups including both client helping groups and administrative task groups, using skills related to conflict management, development of cohesion and consensus, and movement toward group goals, as appropriate for the group. Objective 2.1: Attend and assist with five client helping groups; intervention, support, or education as co-facilitator, or agency intern/representative. (Student may move from

intern/ representative to co-facilitator to facilitator, if appropriate to agency setting and acquisition of skills) By November 1st, I will research the methods of Dialectical Behavioral Therapy used in Haven House Skills Support group. By November 15th, I will research the methods of Todo Buddhist mindfulness used in the community lunch series. By December 1st, I will have attended at least two regular meetings of each of Haven Houses Three Support Groups (Skills Training, Addiction, and Mindfullness). Objective 2.2: Assist and participate in a three community events/meetings; outreach, education, or fundraising as an intern/representative for Haven House. (Student may move from intern/ representative to co-facilitator to facilitator, if appropriate to agency setting and acquisition of skills) Assist in planning, outreach, and logistics for events/meetings. Attend three events/meetings by December 1st. Assist with follow-up and evaluation of events/meetings. Objective 2.3: Participate in a minimum of three administrative groups (team meetings, committee work, etc.). Student will facilitate or co-facilitate these groups or participate in them as a member. By December 1st, I will attend five advocate meetings. By December 1st, I will attend three staff meetings. Indicators: Participation and group skills will be discussed and evaluated after each support group meeting. Meeting/events will be discussed within the Learning Experience Narratives Meeting/event attendance will be documented in weekly timesheets; signed by field instructor. Documentation will be included in learning portfolio. Goal 3: To learn program development by assisting with the creation of a fundraising and community service project. Objective 3.1: Research and Compile Data on Thrift Store Management, Policies and Procedures. By November 1st, Conduct research and compile data from resources online about store policies and procedures. By November 1st, Interview local residents and store owners about thrift stores re: the need and the desires of community. Consult with local and statewide thrift store owners.

Objective 3.2: Create Policies and Procedures for Haven House Thrift Store. By November 15th, research, create, and present a volunteer handbook for Haven House Thrift Store to staff. By December 1st, create and present a Policy Manual for the Haven House Thrift store to staff.

Objective 3.3: Assist With Daily Logistics of Creating a Thrift Store. By November 15th, design and create store layout. By November 15th, manage and process donations By November 15th, assist in raising community awareness and outreach about the store project. By December 1st, attend staff meeting at store to finalize store design and policies. By December 10th, I will assist in training volunteers and staff on policies and procedures of store.

Indicators: Research and presentation of Policies and Procedures will be subject to evaluation and by staff and Field Supervisor. The Volunteer Handbook will be evaluated by staff and volunteers and effectiveness will be evaluated after store opens. Thrift store design and logistics will be evaluated by Field Supervisor during biweekly site visits. Time spent on Thrift Store Project will be documented on weekly time sheets, signed by field supervisor. Documentation will be included in learning portfolio.

In meeting the objectives for these three goals, I will demonstrate the following:

Apply primary practice interventions/approach(es) of the agency (social casework, family-centered services, crises intervention) Develop a code of behavior based on the social work profession, its ethical principles and standards as codified in the National Association of Social Workers Code of Ethics, and practice accordingly. Apply the knowledge and skills of a generalist social work perspective to practice. Demonstrate the ability to perform the role of Advocate at entry-level social work role by attainment of the performance standards associated with the role of Advocate defined in the MSW Field Education Manual. Critically analyze, apply, and integrate content from social work courses with the practicum experience. Assist Haven House to help accomplish its mission.

13. Terms and Conditions. a. No more than forty-eight (48) hours may be credited toward the MSW students total practicum time before a draft of this Learning Contract has been submitted. b. This Learning Contract may be revised with the consent of all parties. Revisions must be in writing and signed by the Field Instructor, Agency Representative (if not Field Instructor), Faculty Liaison, and student. An original is to be sent to School of Social Work in attention to the MSW Distance Field Education Coordinator, with a second original given to the field placement agency. Copies for distribution are specified below. c. The parties agree to adhere to the established policies and procedures of the MSW Field Education Program. These are described in the MSW Field Education Manual. The MSW Distance Field Education Coordinator may be contacted for updates and to address any questions that arise pertaining to policies and procedures related to the MSW practicum. d. The student shall not be employed by the agency during the term of this Contract. 14. Signatures.
