2013 High school / College Snow Jam Brian Head Utah!!!

INFORMATION SHEET
*Space is limited, and it is on a first-come, first served basis. All deposits are NON-REFUNDABLE.

TIME / DATE:

-Leave Thursday afternoon, Feb. 14 from Grace Church @ 2:30pm, for Brian Head! -Return to Grace, Monday Feb 18 around 7pm (Well make every effort to call parents if it looks like we will be early or late at the number you have provided.) *You will need to drop off your stuff in the youth building BY Wednesday the 13th before 5pm. Make sure you have your money/purse/wallet/ID with you. There will be no access to packed bags/gear until we arrive in June. -$285* (non-Refundable $50 DEPOSIT DUE Jan. 6th / REMAINDER DUE BY Feb 3rd) -INCLUDES: Transportation, 4 days/4 nights lodging, all meals once we arrive, 3 days lift tix. -DOES NOT INCLUDE: 3 meals on the road, rentals/lessons, extra spending money, any damage to rooms and/or equipment. * IF YOU REQUIRE RENTAL EQUIPMENT: MAKE SURE YOU MARK ON PERMISSION SLIP/$285 DOES NOT INCLUDE RENTAL IF NEEDED. -Money for 3 meals on the road (Dont pack this in your bag!), all your toiletries (dont need to bring sleeping bags or towels), snow gear, clothes you can layer in case it is hot or really cold. Brian Head is at a high elevation and can reach extremely cold temperatures during the morning and evening, pads are recommended- especially helmets!, MODEST apparel -DONT BRING: Alcohol/tobacco, weapons, drugs, lighters/matches, inappropriate music/magazines, e -Stay in groups of at least 4 people at all times. -No guys in girls cabins and vice versa, you need to attend ALL group meetings and meals. -Listen to leaders, respect others and their property. -Dont damage rooms or equipment- you are responsible. -If you get hurt, go immediately to ski patrol. If someone is unable to move, leave a person with them, and send one or two to ski patrol for help. Notify a leader immediately. Dont move anyone who has a neck injury. -No one is allowed to go out of the designated ski resort area (Ex.- backcountry riding) -Dress in layers, and be ready for hot or very cold. Bring very good gloves and a helmet/hat. Most body heat is lost through your head. Waterproof proof your gear before the trip if it needs it. -Remember sunscreen, lip balm, sunglasses/goggles, etc. Its difficult to ride without goggles when it gets cold, windy or snowy. -Borrowing equipment is not recommended. If you do borrow equipment however, make sure that it has been properly adjusted for your ability level and weight. -The person downhill from you ALWAYS has the right of way. -Always use a leash on snowboards. -Do not attempt to latch your binding on the lift. Its dangerous, and you may be kicked out for doing it. Do not jump off of a lift at any time- same reasons. -Be careful and know your skill level. Start slow- especially the first day. -Skiing/snowboarding off trail is not for beginners, and is off limits for anyone not wearing a helmet.

I am the legal parent/guardian of the student named above. I give my son/daughter/ward permission to attend the 2013 Snow Jam on Feb 14-18, 2013 I understand that skiing/snowboarding is a dangerous activity. In light of this I have signed the MEDICAL TREATMENT CONSENT & LIABILITY RELEASE FORM FOR MINORS. I authorize the adult leadership to approve medical treatment at their discretion as deemed necessary. In the event of injury or accident, the parents/guardians insurance carrier will be the primary carrier for the responsibility. I have read and understand the Tips and Guidelines. My son/daughter/ward has read and understands the Tips and Guidelines on the trip flyer. If my son/daughter is a discipline problem, I understand that I may be required to pick them up or arrange for immediate transportation home at my expense (as judged by the Youth Pastor & Leaders), and that money deposited for the trip is non-refundable. I understand that all deposits are NON-REFUNDABLE, and will not bug the Youth Pastor if my son/daughter/ward changes their mind, gets on restriction, their friend decides not to go, etc.
