Student Matinee Transportation Subsidy

2012-2013 Student Matinee Transportation Subsidy Reimbursement Request Form To receive transportation reimbursement for attendance to a Childrens Theatre Company 2012-2013 Student Matinee you must have applied for and received a 2012-2013 Transportation Subsidy. (Approved applications will have received a confirmation via e-mail.) To request reimbursement following your performance please: 1. Complete the following reimbursement request form. Include the information exactly as it appears on your Student Matinee reservation contract to ensure accurate processing. 2. Submit proof of payment to the bus company for the field trip to Childrens Theatre Company. 3. All Transportation Subsidy reimbursement requests must be received by June 21, 2013. Reimbursement will be made within 30 days of receiving all required materials. 4.
