BAC MINUTES No. 05, SERIES OF 2012 MINUTES OF THE BAC MEETING HELD AT THE MUNICIPAL HEALTH OFFICE (CAMALIGAN PASTORAL CENTER) LAST 6 FEBRUARY 2012 AT 10:15 OCLOCK IN THE MORNING. Eduardo TWG Head Absent None The meeting started at 10:50 A.M. with a call to order by the BAC Chairman. The presence of a quorum was determined after the checking of attendance was made. The reading of the journal of the 1 February 2012 meeting was made and the minutes was approved without any correction, except some comments on the discussion of the other matters as regards the proposed travel to Baguio City on 21-25 February 2012. The BAC approved the canvass and the hiring of a vehicle for the purpose of this travel. The order of the agenda for this meeting followed, to wit: 1. Purchase Requests submitted for the week: a. Mayors Office for the following purchases: i. Ambulance battery in the amount of PhP 5,600.00; ii. 3000 pcs. Waste Segregation posters in the amount of PhP 34,500.00; iii. Nosol tires for LGU Ambulance in the amount of PhP 11,236.00; iv. Vehicle Tire (205/70 R15) in the amount of PhP 13,800.00; v. Tarpualin printing in the amount of PhP 30.055.57; vi. Vehicle repair materials in the amount of PhP 7,810.00; vii. Transport vehicle repair in the amount of Php 4,640.00; and viii. Various office supplies in the amount of 27,625.00. b. Municipal Accounting Office for the purchase of printed forms in the amount of PhP 9,500.00; c. Municipal Health Office for catering service in the amount of PhP 50,000.00 and seminar materials in the amount of 3,895.00 2. Other matters. The first agendum for this meeting, the purchase requests submitted for the week, was discussed by the body. All these purchase requests were recommended by the BAC for shopping using Section 57, 1.b. of the revised IRR of RA 9184. These purchase requests recommended for shopping will be submitted for PhilGEPS posting and canvass only after the BAC had issued the necessary BAC resolutions and which has to be approved first by the Mayor. The BAC shall sign the BAC resolutions for these purchases in their next meeting on Wednesday, 8 February 2012.

There were no other matters discussed. However, there was a reminder for the BAC members to attend the bidding activity for the purchase of medicines on Wednesday, 8 February 2012, at 10:00 A.M. at the MHO, Camaligan, Camarines Sur. The BAC Secretariat shall no longer issue a convocation letter for this activity. After disposing all of its business for the day, the BAC adjourned at 12:00 A.M.
