RULES & REGULATIONS OF FACILITY USE

In case of emergency or for reasons beyond RAVE Soccer Complex LLCs, control, RAVE Soccer Complex LLC reserves the right to cancel scheduled activity prior to the scheduled use without liability. Refunds will be made only if cancellation by RAVE Soccer Complex LLC is necessary. Occasionally it may be necessary to reschedule, relocate or cancel a request previously approved. In this event, the group or individual will be given as much advance notice as possible. If athletic fields will be used for a period of time, all events/ activities and make-ups, if cancellations are necessary, must be completed within the dates and times of use indicated on this application. No refunds will be issued for games cancelled due to increment weather or any other reason beyond RAVE Soccer Complex LLCs control. Make up games may be scheduled free of charge within the dates of use indicated on this application. For each fee based reservation that is cancelled by applicant, a $15 administrative fee will be assessed. If the cancellation is made later than five (5) days before the activity date - the entire fee is non-refundable. All cancellations, adjustments, or rescheduling must be submitted in writing by the applicant or responsible party listed on the Application for Athletic Field Use. Smoking and consumption of alcoholic beverages is prohibited at all times. No exceptions! Participants may access office building restrooms upon RAVE SC's approval at additional cleaning fee of $ 25 for each day of usage. If you choose to pay $ 25 daily cleaning fee, your organisation is not responsible for cleaning. If the participant's organisation does not want to pay cleaning fee, it is responsible for cleaning, and a deposit of $ 50/day is required. Participants are required to clean the restrooms at the end of their activities daily. Cleaning the restrooms includes -

removal of trash and emptying trash containers, cleaning urinals and toilets. Chemicals are only allowed to be used on urinals and toilets. Do not use chemicals on any other areas, but water. Water hose will be provided to clean the floors. RAVE SC will inspect the restrooms daily and if they are not clean, $ 50 will be withheld from the deposit for each day when the restrooms are left unclean. No animals or pets allowed.

No solicitations, flyers, or selling is permitted on the RAVE Soccer Complex grounds or parking lot without written approval of RAVE Soccer Complex LLC regardless of the club affiliation. If field usage is questionable due to weather conditions, I agree to call RAVE Soccer Complex weather hotline three (3) hours prior to my scheduled field use. If the fields have been closed, I will not use the fields. If I, the Applicant, am not the Responsible Party, I represent and warrant that I am authorized to execute this application on behalf of the Responsible Party. I also agree that I will: (1) be financially responsible for any costs incurred by RAVE Soccer Complex LLC for damages to the property; (2) be financially responsible to reimburse RAVE Soccer Complex LLC for reasonable attorney fees to enforce the provisions of any contract that is issued for the activity described in this application; (3) forfeit all fees and deposits as partial compensation to RAVE Soccer Complex LLC for any costs associated with the enforcement of the provisions of the application or reservation; (4) forfeit all fees and deposits if it is determined that I have provided false information on the application; (5) be financially responsible for any RAVE Soccer Complex LLC costs that exceed fees and deposits already collected by RAVE Soccer Complex LLC for enforcement of provisions related to this application or reservation. The Responsible Party agrees to fully indemnify, defend, and save harmless, RAVE Soccer Complex LLC, its officers, agents, and volunteers from and against all actions, damages, costs, liability, claims, losses, judgments, penalties and expenses of every type and description, including, but not limited to, any fees and/or costs reasonable incurred by outside attorneys and any fees and expenses incurred in enforcing this provision (hereafter collectively referred to as Liabilities), to which any or all of them may be subjected, to the extent such Liabilities are caused by or result from any negligent act or omission or willful misconduct of the Responsible Party in connection with its use of RAVE Soccer Complex. This shall be a continuing release and shall remain in effect until revoked in writing.

I hereby attest that the information contained in this application is true and correct. I understand that this is only an application and not a guarantee an activity/event will be allowed to commence. If this application is approved, I agree that: (1) if any of the information contained in the application is found to be false; or (2) should my conduct, or the conduct of any participants or guests, not be as described in the application; or (3) should any applicable City, County, State or Federal rules, regulations, codes or laws be violated, any approval of the application shall automatically become null and void and any activity associated with this reservation will immediately cease. If the activity has not yet taken place, the approval of the application will be cancelled.

Every participant of the event shall read and sign the Indemnity Waiver, Release, and Assumption of Risk Agreement provided by RAVE Soccer Complex. I agree that participants, who have not read and signed this Agreement are not allowed to enter the premises. I also agree to have a Responsible Party (or its authorized representative) present at the premises during the entire activity time to monitor the compliance with the rules of the facility use by the participants.

All debris and trash must be removed from the permitted site immediately after the activity. Failure to do so may require RAVE Soccer Complex to call upon Bay Disposal. All expenses will be the responsibility of the applicant. Refund requests other than the facility deposit will not be returned if the request is made more than 48 hours after your activity. These instances can be reported by telephone at (757) 544-9000 by fax at (757) 464-0150 or through e-mail at admin@ravesoccercomplex.com and further investigated.
