BEFORE HOSPITALIZATION According to the patient being healthy means that you are free from any diseases.

NUTRITIONAL-METABOLIC PATTERN

Before hospitalization the patient likes to eat meat and fishes. He likes also to eat salty foods just like bagoong. He drinks approximately 4 glasses of water a day & 8 oz of soft drinks. When he was diagnosed with kidney stone, he started drinking buko juice 3-4 times a week. ELIMINATION PATTERN Before hospitalization the patient urinates 3-5 times a day with amber color urine. He defecates once a day every noon ACTIVITY- EXERCISE PATTERN Before hospitalization the patient wakes up at around 6:00 in the morning. He performed his daily activities without the help of others. At 8:00 he will go to his work. He considers his work as his exercise SLEEP- REST PATTERN The patients usually sleeps at exactly 9:00 in the evening then wakes up at around 6. He usually takes a nap every 12:00 noon. ROLE-RELATIONSHIP The patient has two children PATTERN in his ex-wife & they are living with their grandparents. His ex-wife is in abroad. His relation with his ex-wife is not good but he continuously supports his two children financially. Actually before his confinement he visited them 2-3 times a month. COGNITIVE- PERCEPTUAL The patient is a high school PATTERN graduate. He graduated at School of Our Lady of AtoCha . he can respond well SELF-PERCEPTION-SELFThe patient is aware of his CONCEPT PATTERN condition but, he did not

DURING HOSPITALIZATION According to the patient he is not healthy anymore because of his condition. And follows all the order of the doctor. During hospitalization he was NPO before his operation & when he recovered his diet was DAT.

During hospitalization the patient was hooked to an in dwelling catheter.

He cannot perform his ADL for the mean time because of his condition

The patient cannot sleep well because of his condition & because of his medication.

The patient's SO was his brother. They have a good relationship to each other. His brother took care him because he cannot ambulate and assist him.

During our interview the patient can respond but he is irritable to answer all our queries. The patient is still aware of his health, he follows what

seek for immediate treatment because of financial problem, and he still eats salty foods. He's not compliant with his doctor's advice. The patient was circumcised when he was 10 years old. he has 2 children in his exwife According to the patient if he has a problem he will just makes himself busy & he will visit his family . or hang out with his friends to drink alcoholic beverages. The patient is a roman catholic. He was baptized when he was exactly 1 year old. He believes in Quack doctors

He doesn't have any sexual activity or intercourse because of his condition During hospitalization the patient sleeps to cope up his stress.

According to the patient, he is praying every night but not active in going to the church.

NURSING HISTORY PRESENT HEALTH HISTORY According to the patient he was diagnosed with kidney stone in the year 2002.He said that he started drinking buko juice every day because he believed that this will cure his disease but later on he just drink buko juice 2-3 times a week. On the mid of June he experienced difficulty when urinating but he just ignored it. Last July 24 he experience pain when urinating so he went to Manango Hospital for his check up but he was diagnosed with Stag horn calculus nephrolithiasis . He was referred at SIGH and he was diagnosed with Stag horn calculus w/ hydronephrosis right kidney, Nephrolithiasis Left kidney & cystolithiasis. Past Health History According to the patient he did not know if he completed his childhood immunization. He couldn't recall how many times he has been hospitalized since he was a kid. He said that he experienced minor illnesses like lbm, cough and colds before. FAMILY HEALTH HISTORY The patient has no family history of Diabetes, hypertension, asthma and other hereditary diseases.
