1) Please provide document control form on cover page, indicate revision, person to prepare, person to approve, date etc. 2) Please provide design check for reference beams and endorsed by PE. 3) Please provide the equipment brochure with certificate. 4) Please provide the LG certificates. 5) Please submit the layout sketch to show setting out of testing equipment. 6) Please provide the excavator operators license, certificates of lifting supervisor, signal man, rigger etc. 7) Please provide the reference drawing (D/ER288/CS/RETW/0001). 1) Please notice that total counter weight should be at least 10% greater than the anticipated maximum test load. (BS 1377 Part 9: 1990 Standards) 1) Please select test location and depth at or adjacent to the location where the real foundation will be constructed, if possible. If the test is performed in a test pit, the width of the pit should be at least 4 to 5 times of plate diameter. 1) More details work activities such as mobilization and demobilization, Lifting operation etc should be considered in risk assessment for plate baring test.
