NSTP DIGITAL PORTFOLIO

This project is a compilation of all your achievements and accomplishments in NSTP. This is a required GROUP PROJECT and equivalent to your FINALS. Please be guided by the following components your output must have: Folder 1: Group Profile (5 points) 1. Accomplish this form Group Name: NSTP Section: ID Picture Name Email Address Mobile Number

2. Leaders Narrative rate the members according to their participation in all group activities (1 lowest; 10 highest). Give a short description on how she/he became part of your NSTP experience (1-2 sentences only). Save the file in word, make sure to put a password (Save As, look for tools, select general options, password to open.) Send the password through Messages in eLEAP. 3. Members Narrative rate your leader according to his role in the group (1 lowest; 10 highest). The documentation officer will collate all data and get the average rating. Each member will write a 1-2 sentence description on how the leader became a part of their NSTP experience, this will be collected by the Documentation Officer as well. Place in one Word file with password. Send the password through Messages in eLEAP Folder 2: Copies of past Forms and other materials used in the actual fieldwork during the 1st semester and 2nd semester (if applicable) such as CDPs, Lesson Activities of the Learners, Evaluations (5 points) Folder 3: High Resolution Pictures (20 points) This folder must contain subfolders of photos from the following NSTP activities; Make sure that the file names of the pictures are your captions (short information about the picture like who is in the photo) 1. NSTP I Fieldwork Photos 2. DRRM Practical Exam Photos 3. Run for Ilog Pasig

4. NSTP II Fieldwork Photos (1st and 2nd fieldwork subfolders) 5. Other NSTP related Activities (meetings, make-up activities, bookdrive, NSTP Week activities etc) Selected photos might be used for the NSTP Photo Exhibit

Folder 4 : Video Documentary (70 points)

2 CD (or DVD) COPIES must be produced (1 for facilitator, one for office) Submission will be accepted only during the schedule of your facilitator. Leaders or any available members may submit. FAILURE to submit the requirements will earn you and the entire group INC in their FINAL grade. Create a cover for the CD with your group name, section and photo of each member. Avoid cramming and produce quality output. This PORTFOLIO is equivalent to your FINAL exam. Do not leave your Portfolio on my table or in my pigeon box, please submit it to me personally for checking, also to assure that the CD/DVD is working.
