Letter of Indemnity
In seeking to participate in this field trip/tour conducted by the organizer that is Uncle Bugs Inventor Academy and the co-organizing team to Honda Malaysia's Pegoh plant in Alor Gajah, Melaka;

I (parents name) _________________________________ NRIC no. ____________________ Hereby agree to allow my children (the participant) to take part in this field trip/tour and also warrant and acknowledge that: 1. The participants general health is in good condition and there is nothing which renders the participant unfit to undertake the field trip/tour. 2. I voluntarily assume all risks to the participant inherent in taking part in this field trip/tour. 3. I acknowledge that the organizer and his/her co-organizing team will take all necessary care for the participant including arranging tour bus transport direct from the departure/pick-up point (main entrance of SJKC Puay Chai 2 bandar Utama PJ) to Honda Malaysia's Pegoh plant in Alor Gajah, Melaka. Nevertheless, I hereby release the organizer and the co-organizing team, family and friends, Honda Malaysia, event coordinators, owner of the facility, guides and representatives, from any unforeseen injuries, accidents or damages towards the participant, in connection with the participation in this field trip/tour, and hereby indemnify, hold harmless and expressly exempt the organizer and the co-organizing team, family and friends, Honda Malaysia, event coordinators, owner of the facility, guides and representatives, from any liability, inconvenience, accident, injury, illness or death to the participant, and any loss or damage to the participants properties and/or belongings arising from any cause whatsoever irrespective of whether such claim or claims arose as a result of the negligence of any person, or from any of the risks, dangers or hazards inherent to the field trip/tour. 4. The participant agrees to obey strictly all reasonable and lawful instructions given to the participant from the guides or their deputies. 5. The participant must always stay in the guides track except where the guides expressly tell the participant not to do so.
