10. APPLICATION FOR STUDENT PERSONAL FINANCE I hereby apply for the Student personal finance facility being offered by MCB Bank Limited (MCB), on the basis of markup, as per terms and conditions set forth herein and in the attached term sheet (which I have read and understood) as well as in the Finance Documentation, including but not limited to Agreement for Financing, which I hereby undertake to sign with MCB.

11. APPLICATION TO OPEN ACCOUNT I wish to apply for an account to be opened in Pak Rupees and maintained at a designated branch of MCB, in relation to the Financing Documentation to be entered into between myself and MCB, subject to approval of my application by MCB. I agree to provide any document which you may request pursuant to my application to open the account. I confirm that I have carefully read and understood, in advance, the terms and conditions of student personal finance account, and further undertake that I will be bound by the same in so far as they are applicable to the account (for instance no cheque book will be issued and no profit will be paid) and such account shall only be utilized by me for the purpose of payment of outstanding liabilities, in terms of the Finance Documentation, and any amendments which MCB may unilaterally deem fit in respect thereto or in respect of any features and services under the account. I hereby agree that the Account shall be subject to the General Terms and Conditions (except as specified here) governing liability/banking account in addition to the terms and conditions hereof. I agree that you may at any time and from time to time at your absolute discretion without assigning any reasons (any requirement whereof I hereby waive) and without notice to me, consolidate all accounts in which I am beneficially entitled and set off the amounts available in any such account against any outstanding amount under financing facility agreement. I agree that you may decline my application to open the account in your absolute discretion without assigning any reasons whatsoever and that documents submitted with application or otherwise shall not be returned regardless of approval or rejection. However, in case of rejection of my application, I shall be conveyed of it, in writing. 12. REPAYMENT OF FINANCING FACILITY I hereby agree that repayment of the subject Financing Facility sanctioned to me, along with payment of mark-up and all other amounts/charges, detailed in the finance agreement, shall be by way of monthly installments, described in the Schedule to the Finance Agreement. 13. IRREVOCABLE STANDING INSTRUCTIONS In the event you accept my application to open an account in my name (the Account), I hereby instruct you to comply with the following irrevocable standing instructions which shall not be revoked by any subsequent instructions unless either you consent to carry out such subsequent instructions or the entire amount of the finance facility, along with mark-up and all other amounts/charges, as stated above, has been paid / repaid to you in full. The above instructions and authority shall be binding on my legal heirs, executors and administrators until MCB has certified that there is no amount outstanding under the finance facility agreement. I irrevocably, instruct and authorize you as follows: i) Repayment through irrevocable standing instructions (ISI)/BBA/DDI In case of sanction of the subject finance facility to me, I hereby authorize you to deduct on the due date (as defined in finance agreement) any of my monthly installment(s) outstanding in terms of the Financing Documentation from my Account to be maintained with you for the subject purpose.

Repayment through irrevocable standing instructions DDA/ST In case of sanction of the subject finance facility to me, I hereby authorize you to deduct on the due date, (as defined in Finance Agreement) any of my monthly installment(s) outstanding in terms of the financing facility documents from my account maintained with you, the details whereof appear in the form.

14. REPRESENTATIONS / WARRANTIES I hereby warrant that no bankruptcy or insolvency proceedings have been commenced against me, that all the information furnished by me in this application is true, complete and accurate and that I have not withheld any material fact. 15. AUTHORIZATION I hereby authorize MCB to exchange / share any and all of my information which comes into its knowledge including the above said information, with third parties, including my bankers for the purpose of, including but not limited to, processing of my application and to know the conduct of any of my accounts with MCB or other banks and with LUMS (whenever so requested by LUMS), so long as I remain a Customer of MCB.

16. LETTER OF UNDERSTANDING I confirm of having read and understood the following clauses carefully, as they are applicable to Student personal loan application. 1. 2. The sanctioning of personal loan is at the sole discretion of MCB. The Student personal loan application may only be processed once the processing fee (and applicable government taxes / levies) of loan amount is paid. MCB may take the processing fee by way of a crossed cheque in the name of MCB or cash deposited in the designated MCB Student Personal Loan Account. MCB will not return the processing fee if my application is rejected. The repayments will be made through my standing instructions regarding the account which will be maintained with MCB for said purpose. The monthly installment will be made on its due date (as defined in finance agreement). I may pre-pay the loan/financing subject to the terms and conditions as laid down in the agreement for financing referred hereinabove. I can be sued/put to trial in a court, in case of deliberate concealment of facts. I will not pay cash to any Bank representative for any charges or services. All payments must be made via crossed cheque or Pay Order in favour of MCB. The rate of Markup on the financing will be six months Kibor+2%. The spread of 2% will remain fixed over the tenor of the loan while Kibor will be reset on six monthly basis. These terms shall be in addition to the terms of the Finance Agreement and the standard terms & conditions provided to me by concerned LUMS Office, which I have already read and understood, before execution.

10. I shall not apply for any sort of loan from any financial institution / bank, without seeking prior permission from MCB until the first installment under the loan/financing has been paid by me to MCB in accordance with the Financing Documentation.

17. DECLARATION I hereby declare and undertake that all information stated in this application and attachment hereto or otherwise provided to you by me or at my request is true/correct and accurate in all respects. I understand that monthly installments are equated for convenience only and deferred payment charges are calculated on reducing balance basis.
