PART IV: STUDENT ACHIEVEMENTS

1) ACADEMIC RESULTS PLEASE SUBMIT A COPY OF ACADEMIC RESULT FOR THE LATEST 3 YEARS OF YOUR CHILD TO WYNKIDS BEFORE THE FIRST CLASS.
2) ACADEMIC & LEADERSHIP

Academically he is doing quite well ,he received awards in 2012 as 2nd in his

class, 5As achievement in chore subjects and one of the ten best achievers in his
school. However he has not shown strong leadership skill.
3) SPORTS

He has not taken any sports seriously but he likes playing football with his brother and cousins.

4) ARTS

He likes to draw and do craft work and was enrolled in an art&craft programme between December 2011 to June 2012 in Kizsports, Great Eastern Mall.

5) OTHERS

He likes reading a lot, the books that he read so far ranges from facts finding such as National Geographic, fiction such as Geronimo Stilton collection and comics.

6) PLEASE DESCRIBE AREAS THAT YOU WANT YOUR CHILD TO IMPROVE. (Note : If the space provided is not sufficient, please attach a separate sheet of paper.)

We would like him to be more communicative, have good interaction with people and strong leadership skill. We would also want him to be more confident with himself and be more active especially in sports.

PART V : TERMS & CONDITIONS

1. WynKids Sdn Bhd reserved the rights to amend the class schedule if deemed necessary.

2. If the student is absent, replacement class will be arranged if medical certificate is presented, at a marginal fee.

3. For the personal coaching and the meeting with parents sessions, a reminder letter will be given to your child based on your chosen date and time, one or two weeks before the session. No replacement will be made if the session is missed.

4. Deposit is not refundable and non-transferable across program.

5. Full payment must be made within 1 week from the date the deposit is paid.

6. Crossed cheque should be made payable to WynKids Sdn Bhd.

7. In the event that students want to withdraw from the program after the program has started, he or she may do so provided all the following conditions are met : a) The students must have completed the camps. b) To notify WynKids Sdn Bhd about the withdrawal no later than 3 days after the camps. If WynKids Sdn Bhd does not receive a withdrawal notification 3 days after the camps, it is deemed that the student will complete the program in the intake that he was enrolled into, regardless of his attendance. c) In the event that refund is applicable, the amount of refund will be calculated based on unfinished hours of the lessons, subject to administration fees.

8. WynKids Sdn Bhd reserves the right to withdraw a student from the program, if it is deemed necessary, by providing a prorated refund within 5 working days after the withdrawal. The decision is at the sole discretion of WynKids Sdn Bhd.

9. In order for graduation certificate to be granted, the students need to complete the program with at least 80% attendance rate.

10. WynKids Sdn Bhd reserved the right to amend the terms and conditions without prior notices.
