PETITION FOR RECONSTITUTION OF TRANSFER OF CERTIFICATE OF TITLE

COMES NOW, petitioner MELINDA RANCHEZ GRUBA, through counsel, unto this Court, respectfully alleges: 1. That the petitioner is of legal age, married, Filipino citizen and residing at 152 E3 MAGINHAWA ST. SIKATUNA VILLAGE, Q.C. where he may be served with court processes, motions, and decision; that respondent BIENVENIDO FLORENDO is the Register of Deeds of PANGASINAN where he may be served with summons and other court processes; 2. That petitioner is the registered owner of a parcel of land located at PANGASINAN and covered by Transfer Certificate of Title No. 21322, Register of Deeds of PANGASINAN, free of any encumbrances; 3. That on 16th of June 1988 the office of the register of Deeds of Pangasinan was burned and all the Torrens Titles in said office including T.C.T. No. 21322 were burned; 4. That said T.C.T. was never mortgaged or sold to anyone. WHEREFORE, it is respectfully prayed that the Register of Deeds of PANGASINAN, issue a reconstituted Original Title based on the owners Duplicate Certificate of Title hereto attached in the name of herein petitioner. 10th day of MARCH in
