I.

Objectives:

General Objectives: After 8 days of student-nurse client interaction, the client will be able to develop a trusting relationship, acquire knowledge about current health condition and promote independence in activities of daily living

Specific Objectives: After 45 minutes of SN client interaction, the client will be able to: 1. state the student-nurses name 2. acquire a sense of reality, restating the correct time, date and place 3. show willingness to communicate openly 4. generate w/ the SN the rules of interaction 5. site the purpose and expectation of the interaction 6. agree as to the time, date and place of next interaction

Setting of Interaction Time: 3pm 3:30pm Date: September 29, 2012 Place: Guizo, Mandaue

III.

Appearance, Behavior, Communication and Implication Appearance Danilo, a 30 year old male, local resident of Guizo, Mandaue was wearing a dirtywhite shirt and black shorts. He has brown complexion with a medium built body. When I first met him, he was standing at a barbeque stall, as observed, he had not taken a bath as he was wearing sort of soiled clothing and his hair was not combed. Behavior During our conversation, Danilo was standing upright with his hands at his side. He maintained good eye contact throughout the conversation and made appropriate facial expressions to emphasize a point. I did not have any difficulty in approaching him and I was responded accordingly. Communication He was alert when it comes to answering my questions. He would sometimes pause for a while and think of what to say. He also asked me some questions in which I answered appropriately.

Implication Danilo was able to verbalize his feelings freely and spoke fluently. Also, he was able to establish good eye contact which is a positive characteristic that he was listening attentively and was more than ready to answer questions.

IV.

Evaluation The objectives set by the student-nurse were executed well and effectively. Attempts to establish rapport with the client was accomplished. The patient was responsive in all the questions of the SN

Communication
