of my antenatal G2P0T0A1L0 pregnant woman in Brgy. Salud at Purok 3, Sta. Lucia, with known diagnosis of asthma suffering from difficulty of breathing when setting position for more than two weeks. She has five history of prenatal last 17th of June 2013. She verbalize her last menstruation was on October 20, 2012, and with the following data assessment; Expected date of confinement on 7th of July 2013, 34 weeks & 2 days age of gestation, blood pressure of 120/70mmHg, pulse rate151bpm, 44kg., Tetanus status: TT1, Fetal heart tone-126bpm on the left lower quadrant abdomen, cephalic in presentation upon Leopolds maneuver checking, and

I did home visit every week for follow up prenatal Gave health teaching imparted to her mother and husband as follows: Instructed to remove allergies causing pets Encouraged to close windows, and outdoor activity when pollen and pollution are their Advised to avoid emotional distress and strenuous physical activity Provide adequate rest for 30 minutes after walking and 8-10 hours of sleeping a day Emphasized to increase fluid intake, and eat nutritious foods such as vegetables, fruits, and milk. I did follow up antennal home visit done last 24th of June 2013after one week she went from the hospital. The vital signs are stable and fetal heart tone is within normal. Referred to outpatient department in the hospital as ordered by her Doctor.

>In my six months in Brgy. San Eugenio, I encountered 9 years old, male pupil, complaint of abrasions on the left foot with minimal bloody discharge and swollen noted cause by dog bite on 6th of June 2013 at 10:39 in the morning, Purok 2. From school covered his wound using white clean towel and immediately went to Barangay Health Station.

I did washed the wound with running water using mild soap for 15 minutes and wound dressing done aseptically. Health teaching imparted to his mother as follows: Encourage to go at Animal Bite Center (ABC) in the hospital for ant- rabies vaccine. Instructed to observe the dog for 10-14 days for any changes in behavior or suddenly die for diagnostic examination. I did home visit on 20th of June 2013 for follow up. His mother verbalize that they send the head of the dog for laboratory and found out negative result. And 2 days after the incident, they gave one anti-rabies to her son. Monitored the vital signs every time I saw him. Referred to Dr. Felix C. Briones, MHO for follow up check up and advice.
