2.1 In the case of death, the following people should be informed the treating doctor: 2.1.1 Police (999) 2.1.2 Medical Director 2.1.3 Director of Nursing 2.1.4 Charge Nurse of In-patient Ward 2.1.5 Relatives of patient 2.2 The body is isolated until the police comes. 2.3 Patient judged to be dead on arrival at the ER(BID) must be certified dead by the doctor on duty and police to be informed (999) and wait to arrive. 3. SCOPE 3.1 Last care for a dead patient in Gulf Diagnostic Center Hospital 4. RESPONSIBILITY 4.1 Physician 4.2 Registered nurse 5. PROCEDURE 5.1. Nursing Record must be completed accurately and timely. 5.2. The doctor who attended the death writes the notes and completes the Death Notification Form. 5.2.1 Copies of the Death Notification Form of HAAD as follows: 5.2.1.1 Death notification with dead body 5.2.1.2 One photocopy of the Death Notification Form is given to the relatives/employer. 5.2.1.3 One photocopy of the Death Notification Form is kept on the patients file. 5.2.1.4 One copy of the Death Notification to police. 5.2.1.5 As per HAAD Standard death must be notified to HAAD. Submission of death Notification electronically has to be within 6 hours of the occurrence of the death event. 5.3 Deceased must have a wrist identification band. 5.4 The body can be released only to the police. 5.5 Care of the Deceased Patients Property

5.5.1 Nursing staff are responsible for the safekeeping of patients belonging and hand over to police on a Patients Property Forms (N 25/1) and obtain signature. 5.5.2 Jewelry that cannot be removed, e.g. wedding ring is entered on the property form with a note to the effect that is still on the body. 6.
