A ROYAL Navy ofcer died after falling 40ft from a landing craft suspended at the side of his warship. Lieutenant Joshua Woodhouse was not wearing a hard hat or harness when he fell. An inquest heard safety concerns had been raised about HMS Ocean two years earlier.
