I am launching the Senior Representative Program for 2009/2010 and am

looking for up to 3 reps from each campus (based on school size) in the tri-
county area (Dorchester 2, Berkeley, and Charleston School Districts).

What’s the Deal?

To start you get:
• One free session on location (up to 3 locations including your
place).
• 10 electronic files for sharing on Facebook, MySpace, etc (with my
logo on them).
• Up to 50 referral cards to share with your friends and classmates.
Each include your portrait, name, information about my studio,
plus a discount of 10% and a free 8x10 to the person that presents
your referral card (card required for discount and free product).
• 10% off regular print orders (canvas prints are 5%) from your
Senior Rep’s Portrait Session.

The referral Bonuses (the good stuff):

- With each paid and scheduled referral, you get $10 cash or $20 print
credit
- Your second referral, you get:
o 1 8x10, 2 5x7, and 12 wallet prints of your choice, one pose from
your Senior Rep. Portrait Session.
- Your fifth referral you get:

o Your choice of a photo skin for either your phone, mp3 player,
or your laptop (photo must be from your session and will
include small text with the studio name).
- Your eighth referral you get:
o An 8x8, 20 page photobook using images from your session
(will include up to 4 pages of digital or scanned images of you
growing up).
- Your tenth referral you get:
o A 36x24 wall poster
o At this point you will have accrued a $100 in cash or $200 in print
credits (plus your 10% discount) and over $400 in products!

The classmate(s) must present a referral card in order to be eligible. All

referred classmates will receive a free 8x10 print from his or her session with a
valid referral card. Your participation in the program will not only introduce
you to a new way for cutting edge, personalized photography, but also a way to
earn incentives towards your session for senior portraits at little or no cost.

The Catch:
Terms and Conditions:
- All photo sessions must be completed by July 26, 2009.
- Parents/legal guardian of the senior must sign this contract and
model release, allowing me to use your images for advertising
purposes and not holding me or the studio responsible for any
damages or injuries incurred in fulfilling this contract.
- If the Senior Rep does not refer at least 2 paying customers by
January 26, 2010 then $100 will be due to the studio to cover costs
associated with time, travel, equipment, and marketing.
- A valid senior portrait card must be presented by a referred student
in order to receive any credits towards prints or products. Credits
will only be issued when a referred senior’s session is complete and
all fees are paid in a timely manner by the referred senior.
- This program is first come first serve and an application must be
filled out before being accepted.

Print a copy of this for your parent/guardian. Minors must either be accompanied by a
parent/guardian or have a signed and dated note from them stating that Emerald Rose
Photography and Mary Peterson will not be held liable for any damages/injuries.

The Senior Representative Program:

I have read and Agreed to all details of the program and agree to comply to
the previous pages in this contract.

Model Release:
For valuable consideration, I hereby confer on Mary Peterson, Photographer,
the absolute and irrevocable right and permission with respect to the
photographs that he/she has taken of me or my minor child in which he/she
may be include:

a)To copyright the same in the Photographer’s name or any other name that
he/she may select;

b)To use, re-use, publish and re-publish the same in whole or in part,
separately or in conjunction with other photographs, in any medium now or
hereafter known, and for any purpose whatsoever, including (but not by way
of limitation) illustration, promotion, advertising and trade, and;
c)To use my name or my child’s name in connection therewith if he/she so
decides. I hereby release and discharge the Photographer from all and any
claims and demands ensuing from or in connection with the use of the
photographs, including any and all claims for libel and invasion of privacy.
This authorization and release shall insure to the benefit of the legal
representatives, licensees, and assigns of the Photographer as well as the
person(s) for whom he/she took the photographs.

I have read the foregoing and fully understand the contents hereof. I
represent that I am the person or parent/guardian of the above named model.
