    PARENTAL CONSENT FORM


    I give permission for my son/daughter to be filmed for and feature in a short film for A Level Media Studies students from Bridgwater College. This film will be assessed by an examiner, uploaded to YouTube (or other video sharing sites) and may be placed on university applications and am fully accepting of these facts. I agree for my child(ren) to be filmed inside and outside of the school environment in their own time. In the shoot outside of the school grounds I agree that either myself or another appropriate adult will accompany them for this period of filming. I agree to the video recording of my child and agree that you may tape and photograph him/her, and record his/her voice also including the right to use their name.
