INTRODUCTION This handbook was developed for use by students and employers as they participate in The Bataan Peninsula tate !niversity on"the"#ob"training program$ It is intended to improve communications and understanding of e%pectations between the employer, student, on"the"#ob training coordinator and company supervisor or trainor$ This handbook outlines the general guidelines, policies and procedures of on"the"#ob training program$ More specific on"the"#ob training re&uirements will be outlined in the course provided by the on"the"#ob training coordinator or company supervisor or trainor$ ON-T E-!OB TRAINING OR SUPERVISED INDUSTRIAL TRAINING 'n"the"#ob training or supervised industrial training is one of the best training methods develop by the university to students because it is planned, organi(ed, and conducted at the company or industrial site$ )enerally, it is the primary method used by the university to further develop the skills of the student"trainee as well as increased their productivity$ !sing the state of the art e&uipment offered by the industry is an opportunity to &ualified student" trainee that enhances their ability to learn various tasks$ BENE"ITS O" T IS PROGRAM TO STUDENTS *pplying and developing knowledge and skills in an actual work setting$ )aining work e%perience relevant to their academic degree$ Developing interpersonal and social skills re&uired in work setting$ Increasing awareness of personal strengths and limitations as a professional$ +elping clarify career goals$ ,inking with potential future employers$ *ssisting in financing their education through an e%tensive paid work team$

ORGANI#ATIONAL STRUCTURE O" T E ON-T E-!OB TRAINING PROGRAM DUTIES AND RESPONSIBILTIES A. Dea$ /$ Determines the &ualifications of students to undergo 'n"the"0ob Training in terms of sub#ect prere&uisites$ 1$ 2valuates, recommends, and submits to the office of the 3ice President *cademic *ffairs the list of students who will undergo on" the"#ob training$ 4$ Directly supervises the coordinator of on"the"#ob training on matters pertaining to all aspects of the on"the"#ob training program5 cooperating company, company supervisor or trainor, schedule of deployment of students, etc$ 6$ ubmits the recommendations of the possible industries to be involved in the on"the"#ob training program to the office of the vice president for academic affairs for her approval$ 7$ ubmits the list of successful student"trainees who finished on"the" #ob training or supervised industrial training for possible #ob placement to various companies or industries$ B. O!T C%&&e'e C%%rd($a)%r /$ 1$ Prepares and submits to the College Dean a Program of *ctivities for the duration of on"the"#ob training$ 'rient the students on the details of on"the"#ob training such as on the general atmosphere of the industries where they will be assigned, personalities of company supervisor or trainor, kinds of co"workers that they will meet the staff of the industries, etc$ Conducts seminars to refresh the students on the different aspects of on"the" #ob training activities that they will need in training such as pre"orientation process, deployment procedure, monitoring and evaluation, submission of re&uirements, work ethics, team building, leadership, etc$ ubmits to the College Dean list of students who are &ualified to take on"the" #ob training together with their company.s preference$ Coordinates with the Company supervisor or trainor where the student" trainees will be deployed to ensure smooth entrance to said company$ 'rients the Company supervisor or trainor on the !niversity.s 'n"the"0ob Training Program of *ctivities such as on the schedule of visits, deployment, seminars to be attended and actual training of student"trainees$ +olds at least monthly conferences with student"trainees to have a follow"up of their activities, their #ob performances and discusses issues and concerns related to on"the"#ob training$ :2mergency meeting can be called as the need arises$; Conducts regular visitation of the industries to monitor the performance of

the student"trainees$ Prepares all pertinent documents related to the on"the"#ob training program such as evaluation forms, observation>monitoring forms, report of grades, record of day"to"day, weekly or monthly activities of the student"trainees, etc$ ubmits on"the"#ob training accomplishment report to the College Dean copy furnish the office of 3ice President for academic *ffairs$ Prepares communications related to on"the"#ob training program noted by the College Dean and the 3ice President for *cademic *ffairs and approved by the !niversity President$ C. C%*pa$y S+per,(-%r: /$ *pproved the recommended but &ualified list of student"trainees re&uested to be assigned in their companies$ 1$ Coordinates with the university.s on"the"#ob training coordinator with regards to the re&uest of the university where the student" trainees are to be assigned$ 4$ 'rients the university on"the"#ob training coordinator about the company policies with regards to on"the"#ob training program$ 6$ *ssists and advises the university on"the"#ob training coordinator on matters affecting the on"the"#ob training program$ 7$ Discusses with the university on"the"#ob training coordinator problems related to the performance of student"trainees$ D. 2%plains the vision, history and the policies of the company to the student" trainees$ Introduces the student"trainees to department upon arrival$ Informs the student"trainees on what division they will be assigned and their specific duties and responsibilities$ Introduces the student"trainees to the department or division$ ets the limits of the student"trainees involvement and responsibilities in the department$ Makes periodic assessment of the performance of the student"trainees in relation to their on"the"#ob training program$ Discusses with the student"trainee the observations made regarding their comments and suggestions for student"trainees further improvement$ Informs the university on"the"#ob training coordinator if the student"trainee is not performing well$ Coordinates with the university on"the"#ob training coordinator where the student"trainees are enrolled$ *ssigns student"trainees to the best &ualified foreman$ 'rients the student"trainees on the services that the company may offer or service areas such as medical and dental clinics, media center, library, sports facilities, and others$ )uides student"trainees in the preparation of various forms re&uired by the on"the"#ob training program$

GUIDELINES "OR T E ON-T E-!OB TRAINING OR SUPERVISED INDUSTRIAL TRAINING E5PERIENCE T1e O$-)1e-!%b Tra($($': A T1ree 3ay Par)$er-1(p :The !niversity, tudent, Cooperating Company; 6.7 INTRODUCTION The on"the"#ob training or supervised industrial training is an essential sub#ect in all programs offered by the Bataan Peninsula tate !niversity$ The on"the"#ob training or supervised industrial training provides the student with the opportunity for professional e%perience in his>her area of speciali(ation$ It has a specific re&uired number hours depending on the program enrolled, and be taken either on the first or second semesters or even summer$ During the on"the"#ob training, the student is provided the opportunity to apply knowledge and skills to practice$ The on"the"#ob training is based on the philosophy that course work and theory, when supplemented with a variety of observations and hands"on work e%perience, will provide the student with the opportunity to enhance his>her professional development and increase understanding and appreciation of the profession$ The on"the"#ob training is a bridge for the student between the academic preparation and the professional future$ It is a three"way partnership between the student, the on"the"#ob training program, and the university$ *ll parties in the relationship assume definite responsibilities, perform specific functions, and achieve benefits as a result of the involvement$ 8.7 OB!ECTIVES The ma#or ob#ectives of the on"the"#ob training are summari(ed as followsE /$ 1$ 4$ 6$ 7$ 8$ 9$ <$ To e%pose the students to the various aspects of industrial practices and work ethics$ To apply the training knowledge and skills for the preparation of their final year of their chosen program$ To enable the students to gain e%periences, relate and apply the knowledge and skill ac&uired in academic settings to real industrial situations and problems$ To give the students opportunities to use new technology as it is use in company$ To enable the student to develop new skills in leadership, supervisory and administrative roles$ To ac&uire skills of the various operations, process and techni&ues used in company$ To facilitate the development of professional work ethics$ To allow students to enhance their personal characteristics through work

REPORTS DETAILS AND EVALUATIONS

ection 97$ Ter*- a$d C%$d()(%$-$ The annual premium per student is fifty pesos :Php7?; sub#ect to change and approval of the Board of Aegents$ The total premium collected shall be put in a Trust -und and shall be managed by an 2%ecutive Committee for that purpose$

ection 98$ C%,era'e$ The Program covers all kinds of accidents including drowning, dog bites, snakebitesWtwenty"four hours a day for 487 days a year, in and out of the school premises$ ection 99$ E00e/)(,()y$ 2ligibility to avail of the Program shall take effect upon payment of the premium$ ection 9<$ Be$e0()-$ The following shall be the percentage amount to be paid by the !niversity to students within one :/; year from the date of accident>death$ *mount to be paid shall not e%ceed one hundred percent :/??F; of the applicable Principle um specified in the Policy chedule$ In case of loss or any permanent disability not mentioned above, the !niversity will adopt a percentage of disablement, which in its opinion is consistent with the table of benefits specified above$ The occurrence of any specific loss or losses for which total insurance shall amount to at least fifty percent :7?F; of the Principal um under this part shall at once terminate all coverage under the Policy, but such termination shall not pre#udice any claim originating out of the accident causing such loss$ If a student, after being in#ured becomes totally and permanently disabled within one hundred eighty :/<?; days from the date of accident, the university shall pay the Principal sum less any amount previously paid as a result of the same accident$ ection 9=$ Med(/a& E.pe$-e Re(*b+r-e*e$)$ If as a result of bodily in#uries or sickness, a student shall re&uire treatment by a legally &ualified physician or surgeon, confinement in a hospital or the

employment of licensed or graduate nurse, the !niversity shall pay the actual e%penses incurred within twenty si% :18; weeks from the date of accident or sickness for such treatment, hospital charges and nurses fees, but not e%ceeding the ma%imum amount specified in the chedule of Benefits as a result of any one accident or sickness$ ection <?$ Da(&y %-p()a& C%$0($e*e$)$ Chen, as a result of in#ury or sickness, a student shall be necessarily confined within a hospital as an in"patient under the continuous attendance of a Physician or urgeon, the !niversity will pay the daily confinement amount stated in the chedule of Benefits for each day that the student shall be confined up to si%ty :8?; days$ ection </$ E.p%-+re K D(-appeara$/e$ If the body of a student covered has not been found within twelve :/1; months after a disappearance, stranding, sinking or wrecking of any means of transportation, death resulting from bodily in#uries shall be presumed to have occurred as provided in the )*++P$ If payment has been released and the student is proven to still be alive, the !niversity shall refund the amount previously released$ *lso, loss caused by e%posure to the elements following a forced landing, stranding, sinking or wrecking shall be covered under the )*++P$ ection <1$ The !niversity shall not be liable in respect to bodily in#ury directly or indirectly conse&uently upon or contributed to by the followingE <1$/ Car, Invasion, act of foreign enemy, hostilities :whatever war be declared or not;, civil war, rebellion, revolution, insurrection or military usurped power, strikes, riots and civil commotionD <1$1 2ngaging in military duty with any armed forces of any country or international authorityD <1$4 Intentionally self"inflicted in#uries, suicide or any attempt at threat, while sane or insaneD <1$6 2ngaging in :or practicing for or taking peculiar to; a&ualung or scuba diving, climbing, or mountaineering necessitating the use of ropes or guides, parachuting, hang gliding, winter sports professional sports or racings other than on footD <1$7 Travel or flight in any vehicle or device for aerial navigation or submarine transportation, e%cept while riding solely as a fare"paying passenger in a fi%ed"wing aircraft provided such helicopter is operated only between

established commercial airport and>or licensed commercial helicoptersD <1$8 Illegal acts of a Person Covered the e%ecutors, administrators, legal heirs, or personal representativeD <1$9 The student is driving any kind of vehicle while the alcohol content of his or her blood e%ceeds the level permitted by lawD <1$< The student have taken a drug unless it is provided that the drug was taken in accordance with proper medical prescription and is not for the treatment of drug addictionD <1$= Illness, diseases, bacterial or viral infection even if contracted by accident$ This does not e%clude bacterial infection that is the direct result of an accidental cut or wound or accidental food poisoningD <1$/? Pregnancy, childbirth or miscarriage$ <1$// Murder or provoked assault or any attempt threatD <1$/1 Aoutine physical e%aminationD <1$/4 Cosmetic or plastic surgery e%cept as a result of in#uryD <1$/6 *ny mental disorder or rest cures$ ection <4$ N%)(/e %0 C&a(*$ * written notice of claim must be given to the !niversity Clinic, within thirty :4?; days after the date of an accident causing in#ury, which may result in a claim$ In case of death, immediate written notice must be given to the !niversity$ ection <6$ S+00(/(e$/y %0 N%)(/e$ The Botice of Claim may be given by, or on behalf, of the student$ The Botice should contain details and particulars that are sufficient to identify the student$ -ailure to give notice within the prescribed period shall not invalidate any claim if it can be proven that it was not reasonably possible to do so$ ection <7$ C&a(* "%r*-$ The !niversity, upon receipt of such notice, will provide the claimants the re&uired forms$ If such forms are not furnished within fifteen :/7; days after the receipt of the Botice of Claim, the student shall be deemed to have complied with the re&uirements of the )*++P as to proof of loss upon submitting within the time fi%ed for filing proofs of loss, written proof covering the occurrence, character and e%tend of the loss$
