CONSENT FORM
For young people under 18 yrs this form must be completed by parent(s), guardian(s) or responsible adult

Skateboarding, BMX, inline skating and scooting are hazardous activities! All members are strongly recommended to wear full protective clothing helmet, kneepads, wrist guards and elbow pads. It is COMPULSORY for Helmets to be worn by all those under 18 years and age if using the Vert. For young people under 18 any yrs this form must be completed
by parent(s) or guardian(s) in the presence of a club leader The CR3 Youth Project (which runs Skaterham) will supervise the park with qualified or accredited volunteer or signed by parent(s) or guardian(s) First Aiders and volunteers but it cannot accept any responsibility for accidents which and countersigned by a of responsible adult witness. may occur as a result the activities of skateboarding, BMX(bike riding), in-line skating and scooting.

I understand and accept that I am/my child is using the park and ramps entirely at my/her/his own risk and, when less than 18 years old, with a parents/guardians knowledge. In accepting this risk I agree to waive any legal entitlement to claim against the CR3 Youth Project, its staff or volunteers, or its landlords for injury or loss or damage to personal property.

May any photographs taken be used either for publicity or displayed at the park? YES.. NO. The CR3 Youth Project is a voluntary organisation.
