PLANNING After 8 hours of nursing intervention, the client will be able to demonstrate behaviors and lifestyle changes to promote healing and prevent complications or recurrence. INTERVENTION > Monitor V/S. > Assess skin and tissues. > Note poor hygiene or health practices. > Inspect lesions or wound daily, or as appropriate, for changes. > Promote optimum nutrition with high protein and calories, vitamins and mineral supplements.
