Nursing Diagnosis Acute pain related to incision of the upper quadrant of the breast secondary to mastectomy

Scientific Explanation o Unpleasant sensory and emotional experience arising from actual or potential tissue damage or describe in term of such damage.

Planning Long term: After 2 weeks of nursing intervention, the client will be free from pain. Short Term: After 8 hours of nursing intervention, the client will be able to: a.) Reduce pain scale from 8/10 to 5/10 a b.) Verbalize atleast 3 divertional activities for pain

Implementation o Determine or document presence of possible pathopsychological/ps ychological causes of pain (e.g. inflammation) o Accept clients description of pain. Acknowledge the pain experience and convey acceptance clients response.

Rationale o To assess etiology/precip itating contributory factors.

Evaluation

Goals were achieved: Long term: After 2 weeks of nursing intervtion, the client was able to be free from pain. Short Term: After 8 hours of nursing intervention, the client was able to: a) Reduced pain scale from 8/10 to 5/10 b) Verbalized atleast 3out of 5 divertional activities for pain.

Objective: o Facial Grimace o Irritated o Not paying attention well o Clutching her breast o Pain scale 8/10

o Pain is a subjective experience and cannot felt by others. o To promote nonpharmacologic al pain management.

o Provide comfort measures (e.g. touch, repositioning, use of heat/cold packs. Nurses presence), quiet environment and calm.
