MEDICAL INSURANCE Provider United Health Care (Oxford Exclusive Metro/Freedom Network) Eligible full-time and part-time regular employees may enroll in a Single, Parent and Child, Couple or Family contract after sixty (60) days of employment. The HLZA medical insurance plan does not provide dental or vision care insurance. To assist with the cost of medical insurance, HLZA pays fifty (50) percent of the cost of the insurance contract; the employee pays the remaining fifty (50) percent through a payroll deduction. 2013 Medical Insurance Rates are as follows: TYPE OF CONTRACT Single Parent/Child Couple Family ANNUAL COST $8,595.12 $15,946.08 $18,910.08 $26,720.76 EMPLOYEES COST at 50% $4,297.56 $7,973.04 $9,455.04 $13,360.38 DEDUCTION PER PAY PERIOD $165.29 $306.66 $363.66 $513.86
TERM LIFE AND ACCIDENTAL DEATH AND DISMEMBERMENT INSURANCE (AD&D) Provider Mutual of Omaha Insurance Company Full-time employees are eligible after thirty (30) days of employment. HLZA pays 100% of the cost of this coverage. Life Insurance - Benefit Amount is 3 times your annual salary, up to $250,000. AD&D - Benefit Amount - the principal sum is equal to the Life Insurance Benefit. LONG TERM AND SHORT TERM DISABILITY INSURANCE Provider Mutual of Omaha Insurance Company Full-time employees are eligible after thirty (30) days of employment. HLZA pays 100% of the cost of this coverage. 401(K) QUALIFIED RETIREMENT PLAN Provider Mass Mutual Retirement Services Full-time employees are eligible after Six (6) months of employment. HLZA provides employees with a 401(k) Qualified Retirement plan which is an excellent means of long-term savings for your retirement. HLZAs contribution, if any, is determined by the employer on annual basis. TRANSIT AND COMMUTER BENEFITS HLZA employees have access to a transit benefit program, allowing employees to pay for certain transit and commuter benefits using pre-tax dollars. Transit Checks are issued on the third Friday of each month. SECTION 125 FLEXIBLE SPENDING PLAN HLZA offers a pretax contribution option for employees. A Section 125 plan is a benefit plan that allows you to make contributions toward premiums for medical insurance, dental insurance, and vision care and out of pocket medical expenses or dependent care expenses on a pretax basis.
